1533662095 2018-08-07 Production Wells No Violations The purpose of my inspection today was a request from landowner John Balik. Mr. Balik would like the well to be referred to the ODNR Orphan Well Program. During my inspection I parked along the northside of the intersection of Oxford and Lough Road and walked to the well to find that it was equipped with 2-inch diameter tubing and rods. There were not any artificial lift systems such as a pumping unit or plunger lift present. No production lines were attached. No production equipment could be located. Mr. Balik stated that there was not any production equipment for the well on his land. The well was situated within a mowed lot roughly 130 feet North from the intersection of Oxford and Lough Road at global positioning system coordinates 40.05600 -81.24567 with an elevation of 1049 feet. The well did not appear to be leaking. RBDMS show that this well was drilled to produce from the Berea with a total depth of 1478 feet. The well was completed in the year 1959 through the open hole shot hole method with 20 quarts of Nitroglycerin.
1548684033 2019-01-28 Production Wells No Violations The purpose of my inspection today was a follow up on an idle well complaint 6056 from landowner John Balik on 8/7/2018. During my inspection the well was equipped with 2-inch diameter tubing and rods. There were not any artificial lift systems such as a pumping unit or plunger lift present. No production lines were attached, and no production equipment could be located. The well did not appear to be leaking and while utilizing the Ventis Pro 4 gas detector zero readings of any kind were recorded. RBDMS shows the well listed as Historic with no history of production reports ever being submitted.
