0491538084 2014-08-27 Plug / Plug Back No Violations On the day of my inspection I witnessed that Columbia Gas was on site and had retrieved 133 joint of 3 inch 9.2#/ft. seamless pipe at 3126.15 feet and the packer 6.75 feet for a total of 3132.85 feet. Columbia gas was preparing to trip back in the hole with 2.875 inch drill pipe to circulate and set bottom hole plug tomorrow.

0865094427 2014-09-03 Plug / Plug Back No Violations On the date of inspection I was informed by Columbia Gas Transmission representative Terry VanSickle that three plugs were set at the bottom of the well starting at a depth of 3132 feet. Petroset spotted three 500 foot plugs from 3132 feet to 2632 feet, from 2632 feet to 2132 feet, and from 2138 feet to 1632 feet. The open hole size plugged was assumed to be 6.25 inch diameter. Petroset pumped 100sk for each plug of Class A cement at 15.6ppg, 1.2 yield, with 5% salt, and 0.25lbs/sk of flake. Columbia will attempt to pull the 6.625 inch casing out of the well the following day and continue cementing after tagging the plugs on 9-5-14. The plugging was not witnessed due to a meeting.
1290679236 2010-11-24 Storage Well No Violations Inspection revealed that well is equipped with 10 conductor and 5.5 casing and 3.5 casing there is evidence at site that well was recently worked on and is currently not on line.
1409849074 2014-09-04 Plug / Plug Back No Violations On the date of inspection I was informed by Columbia Gas representative Terry VanSickle that the 6.625 inch casing had been mechanically cut at a depth of 585 feet. They would be pulling the 585 feet of casing out of the hole today using a cutting torch to separate joints. Columbia has Petroset scheduled for the following day to cement from the surface casing to the top of hole.
1409945523 2014-09-05 Plug / Plug Back No Violations On the day of my inspection I witnessed that Columbia Gas had retrieved a total of 578.15 feet of 6.625 inch casing and tagged last cement plug at 1719 feet. Columbia Gas pulled up to 1709 feet and Petroset was on site and gained circulation and pumped a 500 foot gel spacer. Columbia Gas pulled 22 joints of drill pipe and Petroset gained circulation and set a 100 sack 50/50 Poz cement plug from 1209 feet to 709 feet. Columbia Gas pulled 15 joints of 2.375 inch drill pipe and Petroset broke circulation and set a 275 sack 50/50 Poz cement plug from 709 feet to 209 feet. Petroset ran out of cement and went back to shop to reload pod. Columbia Gas ran 7 joints of 2.375 inch drill pipe and tagged up at 217 feet and Petroset gained circulation and set a 50 sack 50/50 Poz cement plug from 217 to surface. Columbia Gas pulled drill pipe and cement stayed at surface.
1795771847 2015-02-04 Final Restoration No Violations On the day of my inspection I witnessed that the site passed final restoration according to ORC 1509.072B.
