0028075758 2010-02-12 Production Wells No Violations Inspected site along with owner Jeff Miley,well is equipped with 6 visible and 2 tubing and valve was in open position on the day of my inspection,owner needs to get well properly registered and placed on his bond and start reporting production on well.
0100300652 2015-10-06 Production Wells No Violations On the day of my inspection following up from a phone call from Lee Robinson representing East Guernsey County Schools where he requested an update on the status of the well. I stated to Mr. Robinson that he would have to contact Information Officer Steve Irwin for a copy of my report. Mr. Robinson stated that they had an agreement with Jeff Miley of Miley Gas Co. to plug said well. Upon arrival I witnessed the well was equipped with 14 inch conductor, 6.625 inch casing and 1 inch tubing with valves in the open position. Identification was present and production records were current. I placed call to Jeff Miley and he stated that a plug permit has been applied for.
0424504568 2016-06-10 Plug / Plug Back No Violations On the day of my inspection I witnessed Miley Gas Co. attempt unsuccessfully to trip 1inch tubing below the packer set at 1400 feet. MGC has attempted to get past packer since Tuesday 6/7/16 tubing was landed at 1400 feet. Cemenco Services Inc. was onsite gained circulation and pumped a 600 foot 90sack Class A bottom hole cement plug from 1400 feet to 800 feet. Cement was displaced with .5bbls of fresh water. MGC tripped 1 inch tubing to 400 feet and Cemenco gained circulation and pumped a 400 foot 75sack Class A surface cement plug from 400 feet to surface. MGC tripped 1 inch tubing out and Cemenco topped off cement with cement staying at the surface. Instructed operator to cut casing off 30 inches below grade and install metal plate identifying well and reclaim according to ORC 1509.072B.
0487525730 2016-06-07 Production Wells No Violations On the day of my inspection I witnessed that the Appalachian Well Surveys was onsite along with Miley Gas Co. AWS located the open hole packer set at 1400 feet and shot the 2 inch tubing off at 1387 feet. Miley Gas Co. was unable to free up tubing. AWS shot tubing off again at 1370 feet and Miley Gas Co. was able to free up tubing. Miley Gas Co. tripped 2 inch tubing from wellbore and AWS perforated the 6.625 inch casing with 6 shots from 1051 feet to 1045 feet, 6 shots from 396 feet to 390 feet and 6 shots from 206 feet to 200 feet. AWS rigged down and Miley Gas Co. began tripping 1 inch tubing back in the wellbore with plugging scheduled for Thursday morning.
0664678347 2017-06-12 Final Restoration No Violations On the day of my inspection I witnessed Miley Gas Company cut casing off 30 inches below grade, install identification plated, backfill, seed, mulch and remove remaining fittings from location. The site passed final restoration according to ORC 1509.072B.
0672467919 2016-06-06 Production Wells No Violations On the day of my inspection I witnessed that the 1 inch tubing has been tripped out of the wellbore and due to the rain perforating of the casing has been rescheduled for tomorrow. Plugging schedule was reviewed with Jeff Miley of Miley Gas Co. which will commence after perforating of the casing.
1465821154 2016-06-13 Plug / Plug Back No Violations On the day of my inspection I witnessed the pit was filled, location had been graded and cement remained at the surface. Miley Gas Co. will complete reclamation in July with placement of erosion control measures and cutting off of the casing.
