0142829928 2010-03-31 Production Wells No Violations Inspected site along with well owner Jeff Miley and inspection revealed that well is equipped with 6 surface casing and 2 tubing(dry gas well) valves at wellhead were in open position on this day.

Well owner was informed that he needs to get wells placed on his bond and start too report production to the division.

0189207678 2015-06-08 Production Wells No Violations On the day of my inspection accompanied by ODNR Inspector Austin Gutridge following up on NOV-1361808104, we were unable to locate well using shovels, probes, handheld GPS units and coordinates which were found in the database.
0716592881 2015-05-19 Production Wells No Violations I received a call at 15:45 on Thursday May 14, 2015 from Jeff Miley stating that they were unsuccessful finding the wellbore when they excavated with a backhoe. Miley Gas Company dug a 12 foot deep hole and then followed the 1 inch pipe northeast until the pipe ended. I stated that both we would have to try to locate the well since it appears that it has slipped and for Miley Gas Co. to fill in area that was excavated. On the day of my inspection following up on NOV-1361808104 accompanied by landowner Tom Yeckley, I witnessed that the hole has been filled and walked the area finding no evidence of the wellbore. I stated to Mr. Yeckley that we would be out with a metal detector to try and locate the well.
0781505455 2013-02-21 Production Wells No Violations Inspection revealed that well is not producing at this time, local inspector will call owner and inquire why well is not in production.
0923660988 2015-08-26 Field Inspected, Well Not Found No Violations On the day of my inspection following up on NOV-1361808104, I met with Jeff Miley of Miley Gas Co. and Tim Hall of Hall Excavating and discussed where excavation was to take place. After 5 hours of excavation using coordinates and a handheld GPS unit did not uncover the wellbore. MGC removed all exposed flowline and the drip. MGC was instructed to seed and mulch the site.
0954501243 2015-01-29 Production Wells No Violations On the day of my inspection following up on NOV-1361808104, I witnessed that the well was equipped with 2 inch tubing which was bent over and broken at the base of a 3 foot by 4 foot concrete pad. I contacted Jeff Miley and he is planning on trying to swab well and get in back into service for possibly transferring to the land owner.
1022549236 2015-05-14 Production Wells No Violations On the day of my inspection following up on NOV-1361808104, I witnessed Miley Gas Co. was on site they removed the concrete slab and were making repairs to the 2 inch and 1 inch tubing as it was parted. Miley Gas Co. had to go get a backhoe so that they could dig down, cut and thread the tubing and begin to swab on the well to try to bring the well back in compliance. If they are unsuccessful Jeff Miley stated they would be applying for a plug permit.
1361808104 2013-02-25 Production Wells Violations Noted On the day of my inspection I meet with the land owner Tom Yeckley to look at an idle well that belongs to Jeff Miley. At the well site my inspection revealed a cemented area (3x 4) with two inch tubing with a broken valve on the end of the tubing, the divisions production records doesnt show any production recorded for this well. The requirements to meet the divisions standards will be to, make the well capable of commercial production within ninety days, or plugging is required within ninety days, properly identify the well, General Safety, Statement of annual production not filed, Failure to comply may result in Forfeiture of Bond.

1993063077 2013-11-21 Production Wells No Violations On the day of my Compliance Follow-up inspection, I witnessed no change in the status of the well, I spoke with the land owner to see if Mr. Miley had made contact with him to try and resolve the issues of the well, the land owner told me that Mr. Miley did make contact with him but the land owner was instructed by his attorney to tell Mr. Miley unless he was there to plug the well that he was not to come back on his land. I spoke with Mr. Miley after speaking with the land owner and doing my inspection at the well and was informed that the deep rights to the well was tied up in a legal battle between to other companies.
