0107224115 2011-12-15 Drill / Deepen / Reopen No Violations During previous inspections the driller, mud logger and additional staff had been informed that surface casing must be landed 50 below the base of the Berea. I received a phone call from the mud logger that the base of the Berea was at 380 and that they were going to set 388 feet of surface casing. I informed the mud logger to hold activities until I was on site and able to review samples. The mud logger had taken very clean samples and the top of the Berea was at 288 KB and the bottom was 380 KB. KB stands for Kelly Bushing, which is 18 on this rig. I informed the driller that another Kelly must be drilled because the surface casing has to be set 430 or deeper. I informed the driller to contact me 2 hours before they are ready to cement surface.
0128062531 2011-12-30 Drill / Deepen / Reopen No Violations Nomac Rig 14 is not currently drilling due to work being completed on the tophead drive. When repairs are completed they will drill another 320 and then start coring operations. The owner is going to take 90 cores. During the drilling of the 5349 hole, fluid was encountered at 4483 and 4683. A lost circulation zone was encountered at 4710. Gas was encountered between 3800 and 3900. The site was stable and drainage controls were in place. Air packs were being removed from location. Spill Prevention Controls were in place and in good repair. Rock lined drainage channel was also in good repair. Security was on location.

1320843107 2011-11-07 Surface Facility Construction No Violations Operator is cutting location. Filter socks have been placed around location. Road apron is under construction. Operator has installed culvert per county engineers specifications. County Engineer is on location. There was off site sedimentation happening at the time of inspection. Brown water was flowing from the apron south along Newcomb Road. Sediment was from the culvert construction. Operator had to install the culvert 3 below grade. This put the culvert below the grade of the ditch causing a dam. Operator was cleaning slurry of soil and water out of the ditch in preparation to install an additional section of cement culvert. Operator is going to install rock check dams along the drainage ditch to prevent offsite sedimentation.

The cellar is full of water and is surrounded by a fence. The disturbed area around the pad had vegetation established. The containment dike around the pad has two openings, one on each side for water run off. The drainage ditches around the location had rock dams in place. The proper identification is at the beginning of the lease road. The lease road and pad is in stable condition.

Express Energy is on site and has completed drilling the conductor hole. Conductor hole total depth was reached at 80. They set 80 of 20, 96 pound new conductor pipe.
1325252814 2011-12-29 Drill / Deepen / Reopen No Violations The rig has stopped air drilling at 5349 to load hole and start drilling on fluid. They started reading 40 units of gas and shut well in to load the hole. Various valves had frozen and the rig operator had to thaw out the equipment and load the hole through the choke. The rig operator stated they had hit zones that made water, zones of gas and zones where they lost circulation. If they picked up the drill pipe they regained circulation. The last gas zone was below the Clinton. The rig operator is going to trip out of the hole today and log the hole. I reminded the driller to keep Spill Pprevention Controls clear of fluids. Drainage controls around the location were in good repair. Farmer has been driving over recently seeded areas to fertilize fields. Security was at the gate. The rig will start coring operations this weekend.
1325609429 2011-12-09 Drill / Deepen / Reopen No Violations On site for the cementing of the conductor casing. The rig drilled a 26 diameter hole to a depth of 80, in bedrock. The rig set 80 of 20 R-3 casing with welded joints. The cement operator ran water ahead and mixed cement. After displacing 12 barrels of fluid, the conductor pipe started to lift out of the hole. The rig operator used an excavator to hold the pipe down. The cement operator attempted to break circulation, but the pipe continued to come out of the hole , lifting the excavator off of the ground. The cement head weld then failed. After repairing the cement head the cement operator again attempted to break circulation. Cement head failed again. Once the head was repaired, the cement operator attempted to circulate and lift the pipe out of the ground to clean hole. However, the conductor pipe locked up in the hole. The rig operator and owner will evaluate the situation and possibly have a crane on location to pull the conductor pipe. I informed the rig operator to contact me when they are ready to cement. The rig operator informed me that job will be in the morning. They contacted me at approximately 1:30 AM and wanted to grout pipe with a tremie tube. I approved the grouting of the conductor.
1325807907 2012-01-05 Plug / Plug Back No Violations I was on site at 2:20 AM for plug back. The operator had contacted me the day before and stated that the job would start between 2:00 and 3:00 AM. I informed the operator, during that phone call, to contact me if plans change. When I arrived the operator had not started into the hole with the drill string. I requested the cement contractor to contact me one hour before cementing starts. Left site.
1327105630 2011-11-14 Surface Facility Construction No Violations County engineer on site. No company representative on location. The rock check dams have been installed in the drainage ditch and the rock apron for the lease road has been completed. The excavated area has been covered with mulch netting. Approximately 100 of lease road has been constructed. All drainage controls are in place.

There was standing water along the north side of the location and it is divided into two areas, each approximately 4 inches deep. I tested both sections of this ponded water for Clorides. The west area - 172 PPM. The east area - 617 PPM. I also tested the fresh water tank on location - 28 PPM. Allowed company to empty fresh water tank on location. Both the tested areas were parking area for the site crew.

1429044520 2012-01-05 Plug / Plug Back No Violations On site for plug back. Operator Total depth of the well is 6210 Hole is an 8.75 inch hole. Operator is going to plug into the intermediate casing. Intermediate was set at 1440. Plugging will be completed in 5 stages. Plugs will be in 978 foot lengths. 16 stands of pipe will be pulled for each plug, the 17th stand will be pulled up then brought back down to desired depth. Good circulation obtained before each plugged was set. Well was plugged back to 1291.
1501086571 2011-12-14 Drill / Deepen / Reopen No Violations The rig was drilling the surface hole on fluid. I checked the drill cuttings the mud logger was collecting. The site is in good repair. The owner/operator had a contractor on site temporary seeding the affected area. Seeding was completed at at a rate of 150 pounds per acre, using a mix of 57.05% Tall Fescue, 19.65% KenBlue 4.700% Pennlanwn Creeping Red Fescue, 11.60% Rymin Ryegrass, 3.05% Companion Ladino Clover, 2.30% Birdsfoot Trefoil, remaining % was other crop.
1514556573 2011-12-16 Drill / Deepen / Reopen No Violations I was present for the cementing of the surface casing. Total depth of hole #2 was 441 KB (Kelly Bushing). The new 13.375 (J-55, R-3, 54.5 lb/ft.) casing was set at 441 KB. The rig operator ran 5 centralizers on the casing, broke circulation, ran/displaced cement. There was approximately 19 barrels of cement returned to surface. The Berea top was at 290 and the bottom was at 380 KB.
1567828121 2018-08-09 Final Restoration No Violations I conducted a final restoration inspection on this site. The wellhead and any associated equipment has been removed. The wellsite has been restored and is currently being used as a crop field. The lease access has been waived and a signed landowner waiver has been received on 6-13-18 by ODNR-DOGRM. There is a plug report on file in the state database that shows the well had been plugged on 11-09-2016. Final restoration passed.

The operator is sampling the cuttings for the landfill. The rig is planning on reaching total depth tonight and logging the hole.
1636288361 2011-12-22 Drill / Deepen / Reopen No Violations I was present for the intermediate casing cement job. The new 9.625 casing was set at 1440. The operator broke circulation, ran flush, mixed cement to correct weight then pumped and displaced cement. There were good cement returns to surface.
