0246223997 2012-09-10 Production Wells No Violations I observed the unit to consist of a tank battery (TB) and wellhead located west of Rt. 700 near the intersection of Hoover Road., and accessed by a gravel lease road adjacent to a residence. The TB is located approximately 700 feet from the main road, and consists of one, 210 barrel (BBL) production tank, one, 110 BBL brine tank, and a mechanical separator unit. All the unit appurtenances appear to be fairly new, and show little signs of rust. The dike construction appears to be adequate. Dike ballast material appears to be relatively free from weed growth. I observed some oil staining around the tank load out fittings, but did not observe any active leaking or detectable gas vapors. The spacing of tanks and mechanicals appears to be good. The TB is not fenced or enclosed. Identification at the TB is present and in good condition. The tanks are not fitted with spark arrestors, low-pressure-relief valves, or carbon filters. The unit appears to be actively producing. The wellhead is located 80 feet (<100) feet to the southwest. The wellhead is fitted with a pump jack system, which was in operation. There is no fencing or enclosure at the wellhead. There is no identification at the wellhead. There was some oil staining around the wellhead casing and some of the valves, but did not appear to be actively leaking. There were no gas vapors detected on the handheld gas meter (Honeywell PHD6).

0604727534 2012-09-10 Production Wells No Violations The unit is comprised of a two-tank battery (1, 210-barrel production and 1, 100-bbl brine), including mechanical separator and associated valves, and a wellhead with pump jack. The well is currently in production. The wellhead is located 80 feet from the TB. The tank and well spacing is in accordance with 1509:9-9-05. Unit identification is present and legible at the tank battery. The tank battery dike is in good condition. I did not observe any leaks, spills, or other signs of contamination. No violations noted.
0730225069 2013-04-18 Preliminary Restoration No Violations My inspection found the well to be equipped with a pump jack. The annulus valve was open. The tank battery and well are in good repair. The tank battery consists of a 210 BBL production tank, a 100 BBL brine tank, and a mechanical separator. The area of disturbance has been graded, seeded, and vegetation has been established. The proper identification is located on the tank battery. The lease road is in stable condition.

1317644468 2011-09-30 Drill / Deepen / Reopen No Violations On site for fracturing of well. Formation perforated at 4072 to 4090 with 26 shots, 4100 to 4111 with 11 shots, 4128 to 4134 with 6 shots. Zero annulus pressure reported by the operator for the five day test. Superior Well Service was performing the fracturing treatment. Formation broke down at 1188 PSI. Pressures were constant during treatment. Operator was concerned with the possibility of fracturing out of zone. Cement bond in the Clinton and the Packer Shell was less than 50%. Cement bond above the Packer shell was good. No circulation on the annulus during the job. Annulus was static. Initial shut in pressure was 1257 PSI, 5 minute shut in pressure was 1203 PSI. Total slurry volume 81,808 gallons, 500 sacks of sand, 60 gallons WFR-3B (.5 gallon/1000 gallons of water), 40 gallons super OW3 (.75 gallons/1000), 5 IC 100L, 2 gallons OB-FE, 500 gallons 15% Iron Acid. Drilling pits are in good repair. Lease road is stable.
1320619650 2011-09-15 Drill / Deepen / Reopen No Violations The rig drilled a 17 hole to 131 and set/cemented 126 of new 11.75 conductor with 250 sacks of class A cement with 3% CaCl. Approximately 14.5 barrels of cement returns were observed. All conditions were acceptable.

The tank battery was diked and had legible signage. The lease access was stable. No violations observed.
