0207813777 2010-11-15 Drill / Deepen / Reopen No Violations On site for long string cement. Cementing water pH 7. Kilbarger was the driller. Petroset was the cementer. Hole was logged, top of the Lockport was 3602. Operator ran used pipe. There was issues with the pipe tally, total pipe 4244 Last joint of pipe was on bottom of the hole. 4.5 inch pipe will need cut down and collared. Hole TD was 4245 driller depth, logging depth 4256. Petroset broke circulation ran cement, locked plug down at 1500psi. No lose of circulation during cementing. TD at 4256 top of the lockport 3602 total fill up needed: 754 feet. 7.875 inch hole/ 4.5 inch pipe .2278 X 754 / 1.18= 145.56 sacks of cement, operator ran 200. Weighed cement during job, at the start of the job cement weight was 14.8 last half cement weight was 15.2. Good circulation at all times during job. Lease road was stable. Pits were in good repair. Drainage was contained on site.

1291904621 2010-11-30 Drill / Deepen / Reopen No Violations Operator has a swab rig on location is currently flowing the well back. Operator has fracturing tanks on the drilling location and three tanks staged at the tank battery location. Operator has ran a hose from location to the tanks at the tank battery location. Operator was using a tractor with a pump on it to boost the flow of fluid. During inspection there was a line burst. Approximately 2 barrels of fracturing fluid escaped before the operator could get pumps shut down. Operator shut down and replaced section of line. The site had received an inch and a half of rain during the night and during the day. Water was running east and west down the lease road. Leak was on the east side of a rise in the lease road. Fluid flowed into the drainage control area. The amount of water on location made it impractical to attempt a clean up. Water flow was too great and fluid was dispersed too quickly. Requested additional help for the line tender. Lease road was not drivable and it was approximately a 1/4 mile between pump and location and 300 yards between pump and tanks at the battery. Wad Pol had help in rout with an ATV. Reminded employee to make sure he was walking line out into tanks when disconnecting, which he was doing. He was also kinking over lines to prevent spilling fluid while disconnecting. Bull plugs where in the tank valves.

1414421369 2014-10-27 Production Wells No Violations My inspection found the well to be equipped with a pumpjack. The well and tank battery are in good repair. The tank battery consists of a 210 BBL production tank, a 25 BBL poly brine tank, and a mechanical separator. The proper identification is located on the tank battery. The lease road is in stable condition.
1726630436 2010-11-10 Drill / Deepen / Reopen No Violations On site for pit construction, permit has Special Permit Conditions, area of thin Glacial till overlying fractured bedrock. Pits were dry, soil was tight brown clay no fractured bedrock noted. Operator lined pit with reinforced liners. Operator has dug a diversion ditch that directs drainage to the west of the location. Diversion ditch discharges into a low bowl shaped area. Drainage appears that it will be contained on site. Requested the operator install silt fence in a horse shoe shape at the point of discharge to control sedimentation. Informed Brine hauler on site, that they need to use dedicated fresh water hosed when pulling water out of streams. Lease road was stable. Fresh water for drilling will be coming out of Swine Creek.
1791713363 2010-11-10 Drill / Deepen / Reopen No Violations Operator called to inform me that during drilling bedrock was hit at 30. Cementing of conductor was waived. HL #1 17 inch drilled to 40. CND was new R-3 32lb pipe landed in bedrock set open ended.
1946068697 2010-11-12 Drill / Deepen / Reopen No Violations On site for BOP test. Charged BOP to 620psi charged hole to 360psi. First test failed BOP lost too much pressure, 60psi. 2nd test Closed BOP at 620psi charged hole to 420psi, pressure bounced up and down for about half a minute stabilized at 420psi after 15minute test, hole pressure was 395psi. Safety fence around pits was up. Location was stable. Lease road was stable.
