0664021702 2011-04-19 Drill / Deepen / Reopen No Violations On site for surface cement. Operator TD hole at 240 feet. Hole was 11 inch. Per the driller bore hole took water during drilling. Operator was drilling on fresh water. Driller was unable to estimate amount of fluid lost. Approximately 8 inches of fluid from the pit. After TD, operator conditioned the hole for one hour. During conditioning there was no loss of fluid. Operator ran 3 joints of new R-3 24lb 8 5/8 inch pipe. Pipe would not run total length, operator had to spin pipe down. Pipe was still tight. Operator tripped out the surface casing. Operator tripped back in with the drill string to condition the hole. Driller was encountering a tight spots at approximately 112 feet and 171 feet. Fluid loss began again during conditioning and reaming of the hole. Driller was advised by consultant Bob Randall and myself to mud up and condition the hole. Driller was able to ream hole and run pipe after mudding up. Per Bob Rendall fluid lose stopped after driller mixed mud. Per Bob, operator worked on conditioning and cleaning hole for 3 hours before tripping surface pipe back into hole. Operator contacted me the next morning at approximately 12:00am, cementers hooked up to cement head. Unable to attend cement job.
1303330493 2011-04-20 Drill / Deepen / Reopen No Violations On site for status check at 7:15am. Operator completed surface cement at 1:25am. BOP test tentatively scheduled for 12:00pm. Review of the cement ticket indicated that during primary cementing, cement did not return to surface. Operator was advised to tag cement, if operator was unable to confirm cement top operator is to run a bond log. 10:00am driller was unable to tag top of cement. Logging truck was brought onto site to log well. Logging tool set down at 166 feet. There was free pipe at 166 feet. At 4:20PM operator is drilling out cement to run bond log to a deeper depth. 4:45pm operator ran bond logging tool, tool set down at 197 feet, no cement behind pipe. Operator tripping back into hole to drill out remainder of the cement. 6:00pm operator is below 215 feet, below the bottom of the surface casing. Ran 2nd bond log. Top of the cement 184 feet. Perforated pipe from 184.5 to 179 with 12 shots, 180 degree shots. Gun length 11.4 set gun at 167.6 and shot. Operator broke circulation at 100psi. Circulation broke, then stopped. Fluid stayed at the top of the pipe, did not circulate. 4 inches of pit fluid pumped into casing. Pit is 56 feet by 9.6 feet approximate front 1/4 of pit is partially full of cuttings fluid lose approximately 900 gallons. Operator shut down for the night.
1303734717 2011-04-22 Drill / Deepen / Reopen No Violations On site at 5:00pm. Operator has Phoenix Well Service LLC on site to cut 8.625 pipe. During the day I had conversations with the company on how the job was going to go. Company was going to plug through the 8.625 inch pipe. Company was advised to trip tubing or 4.5 inch casing into the 8.625 inch casing start building a plug below the surface casing to below the cut point, trip tubing or casing out, shoot casing, pull 8.625 inch casing and then trip back in and complete plug. Operator was also advised a ready mix truck would also be an option. Operator was completing the job during the evening a ready mix truck was not available. Operator would prefer to plug job on Monday, 1509.12 operator has 24 hours to correct defect casing. Operator had Universal cement in rout to the location. Operator made the decision to shoot pipe off before building the bottom plug and pull pipe. Operator was asked multiple times if they were confident in there ability to trip back into hole to TD after pipe was shoot off. Operator shot off pipe at 173 feet, pulled 4 joints 8.625 pipe. Operator made the decision to trip drill string into the hole to plug with. Drill string sat down approximately at the rip point. Operator tripped out, drill string was plugged with mud. Operator tripped back in with a modified swedge on the end of the string. String hit bottom at the same location. Universal Bulk truck broke down en route to the location. ETA unknown. Project was shut down 9:10pm. Operator will need to mill out hole to TD and plug.
1303920050 2011-04-25 Plug / Plug Back No Violations Operator tripped into hole with driller collars and a 7 7/8 drill bit with the center of the bit cut out for cementing. Operator was able to obtain circulation and thread the pipe into the 8 5/8 casing. Operator tripped 7 driller collars into the hole. Collar lengths were 31 foot, bottom of the collars was 210/ 8 foot KB. Universal Well service on site to plug well. Plug set in two stages. First plug 210 to 86, 4 collars pulled, broke circulation ran cement for 2nd plug. Pit level increased 1 foot, with drilling mud returning to surface during cement job. Cement returned to surface. Cement used for plug job was approximately 3 times the needed amount. Cement returned to surface. Cement feel three feet, operator continued to top off hole until cement stabilized. Operator manual put 75lbs of Flake down the hole during topping off. Operator ran 125 lbs of flake during cementing. Cement stabilized 1.5 feet below top of CND casing.
1727621325 2011-04-21 Drill / Deepen / Reopen No Violations 7:30am operator is mixing mud. Operator brought the mud up to a 40 viscosity. Operator attempted to obtain circulation on the surface casing. No circulation. Operator ran 8 inches of pit fluid down hole. Pit is 56 feet X 9.5 feet, front 1/4 of the pit is partially filled with cuttings. 11:35am operator is missing mud for 2nd attempt, fresh water source is the pond north of the location. Operator was unable to obtain circulation. Well is to be plugged. Operator is unable to obtain circulation. Review of the water wells in the area show that the aquifer and the loss circulation zones are at approximately the same elevation. Continued attempts to circulate drilling mud, can cause contamination of the aquifer. Decision to plug well was made per 1509.12.
