The tank battery is equipped with one 210 barrel production tank, one 100 barrel brine tank and a mechincal separator. Equipment spacing is in compliance and proper identification is posted. The lease road is constructed out of durable material.
1307372640 2011-06-05 Drill / Deepen / Reopen No Violations On site for long string cement. Operator ran new R-3 4.5lb pipe. Pipe set at 4240. Hole was 4258 taken GL 7 and 7/8. Operator conditioned hole before cementing. Broke circulation, ran cement, displaced and locked plug down. No lose of circulation during cementing. Job went well. Pits were in good repair. Lease road was stable. Dave Harcker on site. Drainage controls in place.

1413003819 2012-09-10 Production Wells No Violations The unit is comprised of a two-tank battery (1, 210-barrel production and 1, 100-bbl brine), including mechanical separator and associated valves, and a wellhead with pump jack. The well is currently in production. The wellhead is located 100 feet from the TB. The tank and well spacing is in accordance with 1509:9-9-05. Unit identification is present and legible at the tank battery. The tank battery dike is in good condition. I did not observe any leaks, spills, or other signs of contamination. No violations noted.

Production for this well was current. The tank battery consisted of one 210 BBL production tank, one 100 BBL brine tank, and a mechanical separator, Valves on the separator were open and the chart showed production. The tank battery was diked and had legible signage. The lease access was stable. No violations observed.

2095781894 2012-09-10 Production Wells No Violations I observed the unit to consist of a tank battery (TB) and wellhead located north of Hoover Road, near the intersection of Rt. 700, and accessed by a gravel lease road adjacent to a farmstead. The TB is located approximately 400 feet from the main road, and consists of one 210 barrel (BBL) production tank, one 110 BBL brine tank, and a mechanical seperator unit. All the unit appurtenances appear to be fairly new, and show little signs of rust. The dike construction appears to be adequate. Dike ballast material appears to be relatively free from weed growth. I observed some oil staining around the tank loadout fittings, but did not observe any active leaking or detectable gas vapors. The spacing of tanks and mechanicals appears to be good. The TB is not fenced or enclosed. Identification at the TB is present and in good condition. The tanks are not fitted with spark arrestors, low-presssure-relief valves, or carbon filters. The unit appears to be actively producing. The wellhead is located 200 feet to the northeast. The wellhead is fitted with a pumpjack system which was in operation. There is no fencing or enclosure at the wellhead. There is identification at the wellhead which is in good condition. There was some oil staining around the wellhaed casing and some of the valving, but did not appear to be actively leaking. There were no gas vapors deteted on the handheld gas meter (Honeywell PHD6).
