0545801973 2012-09-10 Production Wells No Violations The unit is comprised of a two-tank battery (1, 210-barrel production and 1, 100-bbl brine) and a wellhead with pump jack. The well is currently in production. The wellhead is located 200 feet from the TB. The tank and well spacing is in accordance with 1509:9-9-05. Unit identification is present and legible at the tank battery. The tank battery dike is in good condition. I did not observe any leaks, spills, or other signs of contamination. No violations noted.
0943587190 2010-07-06 Drill / Deepen / Reopen No Violations On site for BOP test. First test BOP was failed. BOP was leaking at the threads. 2nd test BOP passed. Pressured BOP to 600psi pressured hole to 220psi. BOP lost 5 psi and stabilized. BOP passed 15 minute test. Berea top was 220 feet Berea bottom was 290 feet. Operator has constructed 3 sumps to collect water for drilling. Driller Jim Dixon refused fresh water from tanks during cement job. Water was black and pH appeared to be off. I was not on site for surface cement. Pits were in good repair. Lease road was stable and in good repair.
1147751206 2013-04-18 Production Wells No Violations My inspection found the well to be equipped with a pump jack. The annulus valve was open and plumbed into a pop off valve. The tank battery and well are in good repair. The tank battery consists of a 210 BBL production tank, a 100 BBL brine tank, and a mechanical separator. The proper identification is located on the tank battery. The lease road is in stable condition.
1321899824 2011-11-21 Preliminary Restoration No Violations Well is equipped with a pumping unit. The well site area has been reclaimed. The lease road is stable. The tank battery is lined and is equipped with one 100 barrel brine tank, one 210 barrel production tank and a mechincal separator. The dike has a drain pipe installed and is in good repair. Equipment spacing is correct. There is a plastic line run from the gas meter run drips at the road to the top of the 100 barrel brine tank. This line is on the ground surface in a drainage ditch.
1833611284 2012-09-11 Production Wells No Violations I observed the unit to consist of a tank battery (TB) and wellhead located west of RT. 700, and accessed directly from the orad via an ungated lease road. The TB is located approximately 300 feet from the main road, and consists of one 210 barrel (BBL) production tank, one 110 BBL brine tank, and a mechanical seperator unit. The dike construction appears to be adequate. Dike ballast material appears to be relatively free from weed growth. I observed some oil staining around the tank loadout fittings, but did not observe any active leaking or detectable gas vapors. The spacing of tanks and mechanicals appears to be good. The TB is not fenced or enclosed. Identification at the TB is present and in good condition. The tanks are not fitted with spark arrestors, low-presssure-relief valves, or carbon filters. The unit appears to be actively producing. The mechanicals appear to be in a reasonable state of repair and maintenance. The wellhead is located approximately 700 feet to the northwest. The wellhead is fitted with a pumpjack system which was in operation. The wellhead is not fenced or enclosed. There is no identification at the wellhead. There was some oil staining around the wellhaed casing and some of the valving, but did not appear to be actively leaking. There were no gas vapors deteted on the handheld gas meter (Honeywell PHD6).
1867111427 2018-09-24 Production Wells No Violations I conducted an inspection on this well. During this inspection, the well was equipped with a pumping unit and in production. The tank battery consisted of one 210 BBL tank, one 100 BBL tank and a mechanical separator. The tank battery was diked with legible identification. The lease access was stable.
