0258573315 2017-02-10 UrbanProduction Wells No Violations During this inspection of this well, it was observed that the well was equipped with a plunger lift assembly and the valves on the wellhead were open. The well was fenced with a locked man door. The last reported production was submitted in 2015. The tank battery consisted of two 100 BBL production tanks, one 50 BBL poly brine tank and a mechanical separator. Valves on the separator were open and the chart showed production. The tanks were equipped with the correct lids and hatches and the vent stack on the production tank was plumbed into a 55 gallon charcoal filter, then plumbed up and topped with a spark arrestor. The tank battery was diked and fenced with a locked man door. The proper, legible signage was located at the tank battery and the lease access was stable. No violations were observed.
0671778511 2013-07-12 UrbanProduction Wells No Violations My inspection found the well to be equipped with a plunger lift unit. The annulus valve is open to a pop off valve and plumbed into the production tank. The well and tank battery are in good repair and are surrounded by a chain link fence. The tank battery consists of a 200 BBL production tank, a 100 BBL production tank, a 50 BBl poly brine tank, and a mechanical separator. The production tank is vented through a 55 gallon charcoal filter and into a spark arrestor. The proper identification is located on the tank battery. The lease road is in stable condition.
0952028655 2010-01-12 Preliminary Restoration No Violations Annulus is plumbed into a pop off and pressure gauge is in place. Zero psi on the annulus. Operator was actively solidifying pits at the time of inspection. Two loads of fines have been delivered into the pits. Operator had finished mixing the work pit, soil was place in the center of work pit, excavator was moved onto soil, blow pit mixing was started. 3rd load of fines is going to be brought onto site in the afternoon. Blow pit was starting to firm up with out the 3rd load. Reminded equipment operator to be careful not to tear the bottom of the linear.
1026895205 2010-01-07 Drill / Deepen / Reopen No Violations Pits are in good repair. Operator is drilling on fluid. Drill depth 3450 Site is snow covered with +/- 18 inches of snow. Lease road is open and in good repair. Checked well location with GPS, well location is within +/- 9 feet of staked location, the 9 feet is the margin of error on the GPS unit. Well location came into question by a neighbor. Property stake is present and north approximately 3 feet from the rig.
1242064257 2010-01-03 Drill / Deepen / Reopen No Violations On site for surface cement. Lease road was stable and snow covered. Permit was posted in the dog house. Pit linear were in good repair. Operator drilled surface hole on fresh water mud. Water was from the river south east of the site. Site was snow covered 12 inches or more snow. Silt fence was up. Safety fence was up. Fire extinguishers were in place. No fuel leaks noted. Hand rails were up. Surface pipe was new 20lb pipe. Surface hole was TD at 330, 11 inch hole. Pipe was set at 300/ 310 with landing joint. Fresh water tank was frozen, valves had to be heated, pumps on the cement trucks were beginning to freeze. Operator broke circulation. Operator mixed one bag of gel and a bag of hauls ran before the cement. Operator mixed cement then displaced. Pumps started to freeze during displacement. Cement returned to surface 4 BBL return of good cement returned to surface and held at surface.

Permit was posted in the dog house. Lease road was stable. Safety fence was up. Eric Poulson was the driller on duty today.

1294235011 2011-01-05 UrbanPreliminary Restoration No Violations Well is a rabbit well. Lock combo wasnt working this day, wellhead had recently been painted, annulus gauge was not visible. Operator has the annulus plumbed into a pop-off with a gauge then a line comes off of the annulus towards the tank battery. Wellhead is fenced, with 8 foot fence with 3 string barbed wire. Tank battery has two 100Bbl production tanks, one 1500gal poly tank and a mechanical separator. Dike is in good repair, dike does have two PVC drain pipes in it, both are closed at this time. 8 foot fence with 3 string barbed wire is up around the battery. Man doors are locked. Spark arrestors, charcoal filter and low pressure relief are installed on the battery. Landscaping has been installed around the meter run and at the west end of the tank battery. Wellhead is not visible from the road and is in a wooded area. Area has been graded, seeded and vegetation is established. Erosion and sedimentation is controlled. No lease road gate, drive is primary drive for homes. Identification sign is posted at the entrance of the lease road. Lease road is constructed out of stone, the road is stable and in good repair.
