0123612369 2013-04-03 Production Wells No Violations My inspection found the well to be equipped with a pump jack. The annulus valve was closed. The well and tank battery are in good repair. The tank battery consists of two 210 BBL production tanks, two 100 BBL brine tanks, and two mechanical separators. The proper identification is located on the tank battery. The lease road is in stable condition. The lease road and tank battery are shared with permit number 2-2009.
1278952450 2010-07-12 Preliminary Restoration No Violations Well is a pump jack well. Annulus is open and above ground level. Well site has been graded and seeded. Vegetation is established. Lease road to the tank battery is stable and rocked. There are eight stockpiles of logs and brush and one stockpile of stumps along the lease road. Lease road north of the tank battery there is erosion damage. Erosion is from agricultural fields up slope of the lease road. Sediment is deposited across the lease road. 100 yards east of the tank battery operator has recently repaired the lease road and installed a culvert. Area has been graded, operator needs to repair seed the graded area. ( Maintenance personnel on site at the time of inspection, requested repair seeding.) Remainder of the lease road: sides of lease road have been seeded and vegetation is established, road is stable. Tank battery has two mechanical separators, two 100BBL brine tanks and two 210BBL production tanks in it. 10 feet between separators, 10 feet between production tanks, 32 inches between brine tank and production tank. Individual identification signs are posted at wells and tank battery.

They do report a puff of gas and the smell of gas. 11/17/09 and 11/18/09 no measurable pressure. 11/19/09 annulus valve left open. Submitted report will be kept on file Salem Office.
