0299531960 2009-11-05 Drill / Deepen / Reopen No Violations On site for surface cement. Operator was tripping out of the hole and got hung in the Berea. Operator was having issues with the Berea swelling. At the start of tripping out once the bit was hung operator could go down but not up, Berea then locked up, operator could not go in either directions. Operator did have and could maintain circulation. Operator attempted to turn drill pipe with tongs. Attempting to free drill pipe with tongs resulted in a twisted drill pipe. Operator was going rethink strategy to free hung drill pipe. Will contact when ready to cement surface casing.

Operator was setting up drill collars with a set of jars to go in and retrieve the last 3 joints of drill string and bit. Pits were in good repair. Driller inquired if he could use caustics down hole to make the Berea Sandstone heave to release bit. Request to use caustics was denied.
0687547313 2017-06-01 Production Wells No Violations During this inspection, the well was equipped with a gas fired motor on the pumpjack. Valves on the wellhead were open. The production records for this well were up to date. The tank battery consisted of one 210 BBL production tank, one 100 BBL brine tank and a mechanical separator. Valves on the separator were open and the chart dated 5/3/17 showed production. Tank valves were equipped with flat plugs. The tank battery was diked and had proper, legible signage. The lease access was stable.

1257861114 2009-11-10 Drill / Deepen / Reopen No Violations On site for surface cement. Operator ran surface casing with no issues. Requested 100% extra cement to be ran during job. Operator hung drill string on 11/05/09, had to use jars to loosen and remove drill string. Drill string freed on 11/09/2009. Operator went back in and reamed hole. Petroset cement company on site for cement job. Operator broke circulation, while switching over to start cement, fluid in the conductor was bubbling. Surface hole was gassing a minor amount. Instructed cementer to mix cement heavy at the end of the job. Petroset ran cement, 10BBL return of good cement to surface. Cement was static. Cement head started to leak at the end of job. Driller had cleaned threads on both the cement head and the landing joint. Cement head stopped leaking about 5 minutes after job was complete.
