0038784519 2010-06-15 Drill / Deepen / Reopen No Violations Operator has TD the well. Total depth 3680. Operator is mixing gel and conditioning the hole. Spoke with Eric Paulson, operator had to starch up, was drilling 4 hour sticks last 3 sticks. Operator encountered a fault at 3006, drill string dropped and was almost hung. Formation took fluid, no circulation lose. Pits were in good repair. Safety fence was up. Lease road was stable.
0165169679 2017-02-14 UrbanProduction Wells No Violations At the time of this inspection, the well was equipped with a plunger lift assembly and the valves on the wellhead were open. The well was fenced and locked. The last reported production was submitted in 2015. The tank battery consisted of one 100 BBL production well, one 50 BBL poly brine tank and a mechical separator. The valves on the separator were open and the chart showed production. The vent stack on the 100 BBL production tank was plumbed through a 5 gallon charcoal filter and topped with a spark arrestor. The tank valves were equipped with flat plugs. The tank battery was diked and fenced with a locked man door. The proper, legible signage was located at the well and tank battery locations. The lease access was stable. No violations were observed.

Company is currently renegotiating tank battery location per the landowners request. Pits have been filled. Operator will line tank battery with a re-enforced liner when battery is constructed. Site has field tiles running through it. Lease road was stable. Test annulus with gas meter that is currently being field test by the division. Inserted meter probe directly into the wells annulus readings: CH4 8.75, CO 1026ppm H2S 8ppm. Meter cleared once probe was removed from annulus. No H2S readings around well head, only readings were obtained directly from inside the annulus.

Operator broke circulation ran cement until it returned to surface then displaced. Cement held at surface.
1023520495 2010-06-09 Drill / Deepen / Reopen No Violations Rig is moving on site. Weather 65 degrees light to moderate rain. Operator cut diversion around pit location, during cutting of diversions, two drain tiles were intersected. Drain tiles appeared to be lower than the stream on the east side of the site. Area around well site was agriculture 30 years ago. Tiles in disrepair have been found on well sites adjacent to this one. Operator disconnected drain tile 30 feet west of the pit location, dug a sump and began pumping water into the stream from the sump. Tiles were 50% plugged, tile furthest south of the well site had stagnate water in it. Both drain tile began to flow when disconnected. Wooded area south of well site is a mix of Slippery elm, Pin Oak, Poison Ivey and other bottom land species. Majority of the trees are successional growth ranging 2 to 3 inches in diameter. There are maples, oaks and pine trees in the 20 to 30 year range of growth. There was standing water along the lease road 6 to 10 feet into the woods 2 to 3 inches deep, once drain tiles were opened water appeared to drain from the woods. The land owner Aznaurova has trenched the back yard of the residence and began to install what appears to be French drains in the yard. Yard is hummocky with open trenches and standing water. Back yard and side yard is over grown with vegetation two feet in height. Land owner has pre existing poor drainage. Lease road is stable with 2 inches of soft mud on top. Area has received significant amounts of rain the past week. Permit is posted in the dog house. Tractor with front bucket and road broom on site cleaning road.
1276278041 2010-06-10 Drill / Deepen / Reopen No Violations BOP is in place. The BOP is chained down to the sub base with three chains and lock downs. Control line is installed. 3 inch choke line connection that is at the bottom of the BOP is pointed down in the cellar. First BOP test start 22:55 400psi on the BOP and 200psi on the hole. Test fail BOP loss 40 psi in 15 minutes. Had operator change angle of choke line connection and tighten fittings, bull plug on the choke line was lose and was leaking. 2nd BOP test 400 psi on BOP 250psi on the hole, test fail 40 psi lose of pressure. Operator cleaned screens on the mud pump, flushed hole and tightened all connections. 3rd BOP test 400 psi on BOP and 290 psi on hole, start 23:25 pass at 23:45 . Hole stabilized at 285psi. 900psi on Nitrogen bottle. Fire extinguishers on the light plant, rig and in the dog house are all current. Pits are in good repair. Operator ran a new 7 and 7/8 Smith Bit. Lease road was stable. Hand rails were up. BOP was not tested to 500 psi due to the shallow surface casing.

Five day test showed annulus pressure at 26 psi. Operator is currently negotiating possible relocation of tank battery. Revised drawings will need to be submitted if tank battery location is changed.
1316530194 2011-09-16 Preliminary Restoration No Violations South end of the lease road is paved, remainder of the lease road is rocked to location. Lease road is stable and in good repair with two turn arounds. Lease road does not have a gate as it was not required during pre-site with the landowner. This is a rabbit well and it is fenced with 8 foot green coated chain link with three strand barbed wire and a man door. The fence is 4 feet away from the wellhead in all directions. Tank battery is fenced with 8 foot green chain link, three strand barbed wire and a man door. All man doors are locked. Annulus on the wellhead is open. Landscaping; there are 3 - 5 and 7 - 6 stock planted pine trees around the wellhead and tank battery. Landscaping is in compliance. Well is 56 from the separator. Separator is 14 from the production tank, spacing is correct. Charcoal filter, spark arrestor and two low pressure relief valves are in place on the production tank. Hatch on the production tank is sealed. Area affected by drilling and completing has been graded, seeded and vegetation is established. 20 south of the south west corner of the tank battery there is a 2 diameter depression. This depression is holding water and the Operator has been contacted. Any remaining silt fence can be removed at this time. Site is stable.

Operator broke circulation ran cement, 5 BBL return of good cement to surface. Cement held at surface. Operator is changing the configuration of the site, due to the land owner wanting as little area affected. The rig is set north south, pits are on the west side of the rig. Blow pit will be lengthen to the south to allow for additional pit space. Operator is going to stockpile topsoil behind the barn on the property. The land owner requested any excess topsoil be placed in his yard for leveling, the decision to do this has not been made by the operator yet. Silt fence is up north and east side of the site. Permit sign is up at the entrance to the site. Lease road is rocked to location. Operator added a toping of limestone today for stability. Operator was actively cleaning the road at the time of surface job. Air head was on the rig floor, air head appeared to be in good repair. Pit was in good repair.

Spoke to Warren, they are going to plant trees per HB-278 the landowner will pick where trees belong. Well is in the woods. Operator paved the first 80 of lease road. Lease road is stable and in good repair. Leak around the tubing, per Warren tubing is off center, leaks when pressure is above 900psi. Company is planning on producing well as much as possible to relieve pressure then pulling tubing and straightening it up in the winter to limit damage to site.
1925809660 2010-06-09 Drill / Deepen / Reopen No Violations On site for pit construction. Weather 70 degrees and clearing. Top 4 feet of soil was brown clay, 20 to 24 inches of sand and gravel and the last foot of the pit was dark gray clay. Sand and gravel layer wept water when initially disturbed then stopped. Operator lined pits with a re-enforced liner. No roots or sharp rocks noted. Drain tiles that flow into the pit have been disconnect, discharge of the drain tile is above the water line of the pit and has been plugged, 2nd tile turns south away from location. Silt fence is up around site. North east corner of the site is flooded, operator is going to dig a sump and pump water into the pit. Water was light green/ yellow from pollen. Pollen Forecast for the area is very high.
2061390735 2014-01-03 Administrative Inspection No Violations E-mail received from Warren Shoenfelt dated 06/22/2010 reporting five day annulus test results. The annulus has 26 psi on it after 5 days of being shut in. Frac is scheduled for July 13th.
