0204969218 2011-11-21 Preliminary Restoration No Violations This well is equipped with a pumping unit. The annulus is closed and is plumbed into the flow line. This well was operational during my inspection. The landowner was on site during my inspection. The tank battery is equipped with a mechincal separator, one 1500 gallon poly brine tank and one 210 barrel production tank. The dike is in good repair. The owner/operator has installed a drain pipe in the dike which has a screw in cap and riser pipe on the ends. All areas affected by the well and tank battery construction have been graded, seeded and vegetation is established. The lease road is constructed out of durable material. Proper identification signs are posted. Barbed wire fence has been installed around the tank battery and the wellhead. The tank battery is adjacent to a barn which the land owner uses for equipment storage.
0272487902 2011-06-27 Drill / Deepen / Reopen No Violations Lease road has been rocked to site. Pits were under construction at the time of inspection. Soil was brown and gray clay. Minor weeping of water was noticed at the bottom of the pit, two gallons or less of accumulation. Area has had record rain fall the past three months. Operator packed are of weep with gray clay. Operator will be spudding today. It was explained to the driller, Conductor and Surface hole drilled on fluid. If less than one joint of conductor is needed, contact me for possible waiving of cementing of conductor and setting of conductor. If any conductor is set it must be cemented. Permit was not posted in the dog house at the time of inspection.
0745856540 2011-07-20 Drill / Deepen / Reopen No Violations Service crew on site, preparing for fracturing on Friday. No annulus pressure equipment was on site at the time of inspection. Had rig crew crack open the annulus. There was pressure on the annulus. Contacted company for reported pressures. Per the company maximum annulus pressure was 70psi. Approval to fracture the well was given. 421X .303 X.70= 89.29 psi allowed.

The tank battery consists of a 210 BBL production tank, a 50 BBL poly brine tank, and a mechanical separator. The proper identification is located on the tank battery. The lease road is in stable condition.

Pits are in good repair, operator was injecting water into the discharge line to keep dust down. Site was in good order. Lease road was stable. Operator shut down Friday for the weekend. Drilling resumed at 8:15am this day.
1310130298 2011-07-07 Drill / Deepen / Reopen No Violations On site for long string cementing. Lease road was stable. Pits, operator filled the south end of the pit with cuttings. There was approximately a yard of cuttings on the liner apron and against the berm. The cuttings where dark red in color. Operator pushed cuttings into pit and was advised to build pits with enough capacity and width to prevent this in the future. Operator will solidify the pits. The pipe that the operator ran was 4.5 inch 11.6 lb/ft pipe, used. Operator conditioned hole before cementing. Cement water has a pH of 7. Petroset Ran 210 Sacks of class A cement. Cement was weighed with DMRM scales. Weight was three time during the job, 14.5ppg, 14.6ppg, 14.8ppg and 15.2ppg last half of the job. Good circulation during job. Operator locked plug down with 1500psi. Operator plugged rat hole with 20 sacks of class A cement.

1986449677 2011-07-22 Drill / Deepen / Reopen No Violations On site for well fracturing. Break down pressures was 1504psi. No pressure anomalies noted during fracturing. Annulus gassed and discharged soap suds during fracturing. Annulus had 70psi on it in the am, annulus was blown down before fracturing. Annulus was static after fracturing was complete.
