0291417265 2009-09-14 Drill / Deepen / Reopen No Violations Drill depth 3064 Working on table at the time of inspection. Drilling was idle. Used 4.5 inch pipe on site. Operator was circulation soap on air. Air was blowing soap up over the side of the pit. Soap was flowing back into pit. Called Mike McFadden requested, either a discharge with a better angle on it or some other method to be used to keep soap from blowing over pit. Reminded driller on site that oil from under equipment needs cleaned up. Bail of peat sorbs was on site.

0599617772 2017-12-14 Production Wells No Violations At the time of this inspection, the well was equipped with a pumpjack and the annulus and flow line valves were open. The production records for this well were current. The tank battery consisted of one 210 BBL production tank, one 100 BBL brine tank and a mechanical separator. Valves on the separator were open and the chart showed production. The tank battery was diked and had legible signage. The lease access was stable. No violations observed.

Report of slight smell of gas at the annulus, per the report. 09/23/09 Frac well. 09/24/09 to 09/27/09 no readings. 09/28/09 Remove equipment leave valve open. Complete report will be kept on file, Salem office.
1281346955 2011-07-26 Preliminary Restoration No Violations Affected area from drilling has been reclaimed. There is a brine scald 36 feet X 1 foot going east from the well head. A valve was opened allowing oil and brine to spray from the wellhead. A flow line leaving the well is weeping brine, line needs repaired. Oil spray around the well head needs remediated. Bio-remediation needs mixed into the soil with the brine and oil and area needs repair seeding. Operator will be contacted in regards to the needed repairs. Tank battery is in good repair. Identification sign is posted. Dike is in good repair. 100Bbl, 210BBl and a mechincal separator are in the battery.
1565414382 2009-09-16 Drill / Deepen / Reopen No Violations On site for long string cement job. Dave Harcker from Excalibur on site. Operator broke circulation ran cement. Good circulation during job. Operator latched down plug with 1500psi. Plug held. West pit has been pumped almost dry. Front pit, fluid level is good. Site is stable. Operator had Petroset Cement company drop off bulk cement for the plugging of rat and mouse hole. Operator will hand shovel cement into holes when rig moves off.
1731774407 2009-09-09 Drill / Deepen / Reopen No Violations On site for surface cement job. Operator has been having issues with circulation on surface hole. Operator was able to correct problems on the hole and gain circulation. Operator broke circulation ran 200lb of gel and 50 lb of cotton seed hauls ahead. Mixed cement until it returned to surface, then shut down for 4 to 5 minutes to allow cement to seep into fractures. Cement fell back approximately 18 inches. Operator pumped another barrel of cement then displaced. 22 barrel of cement returned to surface. I had rig hands dam off drainage channel and hold back cement. Cement began to drop after job was complete. Rig hands shoveled cement from channel into space between the conductor pipe and surface casing to keep space full of cement. Cement head began to leak, hands tightened cement head and stopped leak. Rig hands used all remaining cement to keep space between the pipes full. Cement continued to fall back, to an unknown depth. Requested Mike McFadden to tag cement before he starts drilling. Will need to grout space if needed. Per the drilled there was a show of gas in the Berea and the Berea took fluid. Walked river after cement job to make sure no cement was seeping out of the bed rock into the river.
1923859498 2013-03-28 Production Wells No Violations My inspection found the well to be equipped with a pump jack. The annulus valve was closed. The well and tank battery are in good repair. The tank battery consists of a 210 BBL production tank, a 100 BBL brine tank, and a mechanical separator. The proper identification is located on the tank battery and the pump jack. The lease road is in stable condition.
2000152572 2009-09-03 Drill / Deepen / Reopen No Violations Operator lost circulation after drilling out from conductor pipe. Drill depth 62. Operator has attempted to regain circulation by using paper bags and mixing heavy mud.
2009273382 2009-08-31 Drill / Deepen / Reopen No Violations Pits are under construction, soils pits were being constructed in was sand and gravel. Silt fence needs installed north east side of site, at the toe of the slope. Rig was moving on site this day.
