Site was snow covered. Pits have been filled. Called pumper requested an identification sign be placed on the pump jack due to the distance the well is from the tank battery.

I informed the operator to contact me if anything changes. No did not receive a call from operator and I was on- site at 11 Pmcementing of long string. At 2 AM the operator notified me that they were approximately 6 hours from being ready to cement long string. I will return to site when ready to cement. While onsite I inspected long string casing which was new 10.5 LB, R-3 pipe. Pits are in good repair. Ground conditions at the site were extremely soft.

The Rose Run is at 6670 to 6720 A plug was set from 6770 to 6470. The Utica Shale is at 5700 to 5940. A plug was set from 6040 to 5600. The operator broke circulation before each plug was set. The operator will set a single stage packer for cementing of the long string.
0631648022 2012-07-06 Preliminary Restoration No Violations My inspection today found the following: The area affected by the drilling of the well has been graded, seeded and returned to pasture. A mix of grass and native vegetation is starting to become established. There are sections of sparse vegetation on the affected area. I contacted the pumper and requested that, in the fall, repair seeding should be completed on any sparse area. Sedimentation is controlled on location. Area affected by the construction of the lease road and topsoil storage has been returned to agricultural use and soybeans have been planted. The well and pumping unit are good repair. Well has an identification sign on it. The tank battery is located on Agler Road, south of the wellhead. The tank battery has one 210 barrel (bbl) production tank, a 100 bbl brine tank and a mechincal separator. The tank battery dike is sufficient for the capacity of the tanks inside of it. There is a drain pipe located in the south east corner of the dike. Drain pipe is capped and closed. Identification sign is posted on the production tank. The lease road is stable to the tank battery. Equipment spacing inside the tank battery is correct.

Drilled 11hole to a depth of 755 and set 738.65 of surface casing (including a 15 landing joint). Cementing: 738.65 X .2542/1.18= 159 sacks of class A cement. Operator to run 250 sacks. Displacement 718.65 X .0636 = 45.7 Bbls. Operator broke circulation, conditioned hole, ran cement and displaced cement. There were 13 barrels of good cement returned to surface. Pits are in good repair.
1320241859 2011-09-30 Drill / Deepen / Reopen No Violations Drilling at 5190. The water level in the blow pit is good. Operator needs to watch fluid level in the work pit. Once fracturing treatment is completed at the Larks #2 the empty frac tanks will be taken to the Miller #1. The driller was made aware of a small hydraulic oil spill near the rig and a rig hand completed task. The BOP, choke lines and discharge line are correct and in place.

Operator is going to drill the last 300 of the well on mud. Driller stated that the surface cement fell 15 feet. Still visible in the Conductor casing. Operator will top off cement.
