0851899872 2016-03-22 Production Wells No Violations My inspection found the well to be equipped with a pumpjack. The tank battery and well are surrounded by a wooden fence. The man doors on the fences need locks installed on them. The tank battery consists of a 100 BBL production tank, a 100 BBL brine tank, and a mechanical separator. The 100 BBL brine tank has rusted through the side wall of the tank and can not be used. The tank needs to be removed from the location or repaired. The tank load line valves have plugs in them but are not locked. The proper identification is located on the fence of the tank battery. The lease road is in stable condition and is secured with a locked gate.
0995209746 2008-12-22 Drill / Deepen / Reopen No Violations On site for CND job. Operator was setting 35 of conductor. Bedrock was 15 on location. Cement truck on site was frozen up, once valves were thawed out, pump broke down. Cement contractor was going to send another pump truck to site. Waived CND pipe, operator was setting less than one joint, no ground water encountered. CND was landed in bed rock, hole was 12.25. Allowed operator to drill surface hole. Surface to be drilled on fresh water or fresh water mud.

Operator is going to shut down till the first of the year. Requested that pits be fenced with safety fence. Operator started fencing pits at the time of surface job. Lease road was stable and in good repair. Permit ID sign was up.

1233777498 2009-01-21 Drill / Deepen / Reopen No Violations No one on site at the time of inspection. Operator was flowing well back from the frac job. Flow line had ruptured and was spraying nitrogen gas and natural gas across the frac tank. 2nd frac tank that was on site, was facing north/south, valve was cracked open on it. Tank was leaking starch and fresh water across the site. There were three propane bottles next to the leaking flow line. Called company, Jon and a hand from John D arrived on site. Well was shut off and pipe was replaced. Jon, informed me that the leaking tank just had fresh water and starch in it. Tested water, there were no chlorides in it. Operator replaced flow line. Suggested to the company when flowing back a well, to have someone on site at all times.
1247675842 2009-07-07 Production Wells Violations Noted Lease road is in good repair. Lease road gate is not up. Tank battery is in good repair, fencing has not been installed. Fencing must be installed on HB-278 wells before the well is in production. Well is in production. Debris around well head needs removed, (pit liner trash.) Final grading around edges of well site needs completed. Pits need graded. Attempted to call Tim Reilly twice. NOV faxed to Tim Reilly (440) 205-8680 Well needs shut in, until fencing is up. HB-278 requires fencing around tank battery and wellhead to be in place before the well is put into production. Lease road gate needs installed.

Fencing has not been installed. Called Tim Reilly, fencing must be installed immediately. Lease road gate needs installed. Permit to plug has been submitted by company. Explained to Tim, I understand if company is attempting to get what it can from the well before it is plugged, but HB-278 rules must be followed. Chardon Village is HB-278 area.

1493647432 2017-03-13 Plug / Plug Back No Violations I was on location to witness the plugging of this well. Nicks Well Plugging is the plugging contractor for Osair, Inc. The pumpjack had been removed from the well and the rods and tubing had been removed from the well casing. The bottom plug was to be set through 1 1/2 inch tubing but not enough joints of tubing were onsite. Nicks needed extra joints to set the bottom plug. Nicks will return following day with extra joints of tubing to set plug.

1570456862 2016-12-20 UrbanProduction Wells No Violations I conducted an inspection on this well at the request of Bill Stief who is looking to purchase the property and well to convert into a domestic well. The well is equipped with a gas powered motor on the pumpjack. The gas line to the motor had been disconnected and the valves on the wellhead were open. The well was fenced but needed a lock on the man door. Production records were last submitted in 2015 for this well. The tank battery consisted of one 100 BBL production tank, one 100 BBL brine tank and a mechanical separator. The 100 BBL brine tank has holes in it and needs to be removed and replaced. The valves on the separator were open and the valves on the tanks were plugged. The tank battery was diked and fenced with a locked man door. There was proper, legible signage at the tank battery location. The lease access was stable. I contacted Rick Osbourne owner of Osair Inc. about the 100 BBL tank needing removed and replaced and adding a lock to the well man door. Mr. Osbourne informed me that application has been made to obtain a permit to plug the well.

I spoke with Jim from Osair about making the corrections to the dike. The proper identification is located on the tank battery. The lease road is in stable condition.

2138828165 2010-02-02 Production Wells Violations Noted 07/15/2009 a NOV was issued to John D Oil & Gas Company for failing to have well and tank battery fenced. Spoke to Tim Reily from John D, well owner is planning on plugging well. Well needed to be shut in, until plugged or fence was installed. Inspection on 02/02/2010 the well is in production, pump jack is running. Chart is in the meter run. Pictures taken of the pump jack operating, tank battery and chart. Well is in violation of 1509:9-9-05 (2) Prior to placing the well in production, the owner shall install a fence around the wellhead and tank battery/separator and associated production equipment. The correct identification sign needs to be installed on the tank battery. Identification sign shows Great Plains as the operator. Lease road gate needs installed. John D Oil and Gas will be placed on the permit hold list until violations are corrected. Pictures will be sent with this inspection report along with the Urbanized Area permit conditions. Please see #27,#28 and #29 of the Urbanized Area permit conditions. Well is violation of 1501:9-9-05 (2) Prior to placing the well in production, the owner shall install a fence around the wellhead and tank battery/ separator and associated production equipment.
