0029129158 2008-05-09 Preliminary Restoration No Violations Operator has ruff graded the lease road. Lease road is impassable at this time, 1.5 feet of mud on the lease road. MJ oil & Gas on site to work on grading. Site is in poor condition operator will not be able to use a dozer on it. Operator will need to use an excavator to move dirt to get it to dry out. Informed operator to make sure mud and sediment is kept out of the creek.

1339527818 2012-06-12 Production Wells No Violations My inspection today found this well equipped with a pumping unit. The well head is fenced and an identification sign is posted at the well head. The first 200 yards of the lease road is down hill to a stream crossing. The next 100 yards of the lease road, to the wellhead, has a steep grade up to the well, has gully erosion forming on the east side of it and is not stable. The operator will be contacted in regards to this issue. Erosion from the lease road is not entering the stream. The tank battery has one 100 barrel production tank, one 1500 gallon poly tank and a mechincal separator in it. The dike is sufficient to contain the volume of the 100 barrel tank. Equipment spacing is correct in the battery. Identification sign is posted at the wellhead and at the tank battery.
1374670363 2013-07-24 Production Wells No Violations My inspection found the well to be equipped with a pump jack. The annulus valve is open. The wellhead is surrounded by a chain link fence. The well and tank battery are in good repair. The tank battery consists of a 100 BBL production tank, a 25 BBL poly brine tank, and a mechanical separator. The proper identification is located on the tank battery. The lease road to the tank battery is in stable condition.
1456819594 2008-08-05 Preliminary Restoration No Violations Site is in a raw state, lease road is eroding. Site needs graded, mulched and seeded. First 150 yards of the lease road is stable, remainder needs graded and stabilized.
2055295654 2007-11-13 Drill / Deepen / Reopen No Violations On site to pick up paper work from surface job. Operator set surface early in the morning. Lease road was not stable and was impassable by vehicle. I inquired to Ed Schultz from Hall and Horning, what was going to happen if emergency vehicles need to get onto the site. Pits were in good repair. Operator was reminded if they are planning on drilling on air, the discharge of the blow line needs to be 60 from the annular space. Current pit construction would dont allow for the spacing of the blow line. Operator was instructed to obtain a road broom and have the roads cleaned. Operator was moving pipe trailers on to site in the after noon.
