0090236703 2007-11-05 Drill / Deepen / Reopen No Violations On site for grout job. During surface cement, cement didnt return to surface. Operator order a ready mix truck with nine sack grout. Operator had covered up the surface hole with clay over burden. Area around the well head was excavated down to the conductor pipe. A one inch pipe was pushed in between the surface casing and conductor pipe. One inch pipe went approximately 20 down before it made contact with a hard substance in the hole. Operator poured 9 sack grout down the backside of the surface casing. Pour was slow with operator working the grout down with a spade shovel. One inch pipe was left in as a siphon. Grout displaced mud and water from the hole. Once it was determined the hole wouldnt take any more grout,( displacement of water and mud had stopped.) One inch pipe was removed, operator continued to work grout down between the casings. Once the displacement of water and mud from the hole stopped, operator off loaded the remainder of the cement into the excavated area around the well head. Note: During grouting, operator would stop periodically to allow grout to work its way down the hole. 3 yards of 9 sack grout was used for the job.
