0016031664 | 2011-12-01 | UrbanProduction Wells | No Violations | The vent line on the 100 barrel production tank needs to be removed. I will contact the operator on this issue. The man doors at the tank battery were locked. The well site was in good repair. The operator has installed a 1/4 copper line on the load line with a shut of valve on it. The copper line is installed into a fitting at the top of the production tank. I contacted the operator who explained that the purpose of this line is to allow for the expansion of Volatile Organic Compounds (VOCs) in the load line. During warm weather there is a potential that the oil in the load line could expand and damage the couplings. The copper line will allow the gas a place to expand and vent. |
0130178424 | 2013-05-28 | UrbanProduction Wells | No Violations | My inspection found the well to be equipped with a plunger lift unit. The annulus valve is open. The tank battery and well are in good repair and surrounded by a chain link fence. The tank battery consists of two 100 BBL production tanks, a 50 BBL brine tank, and a mechanical separator. The vent stack on the production tank is plumbed into a 5 gallon charcoal filter. The vent stack is then plumbed up and topped with a spark arrestor. The proper identification is located on the tank battery. The lease road is in stable condition. |
0338167104 | 2008-10-14 | Production Wells | No Violations | Well is in good repair. Annulus is open. Tank battery is in good repair. Waiver submitted from landowner for the lease road gate. Well is in the landowners back yard. Well and tank battery are fenced and man doors are locked. |
0364811678 | 2017-02-06 | UrbanProduction Wells | No Violations | During this inspection, the well was equipped with a plunger lift assembly and the valves on the wellhead were open. The well was fenced with a locked man door. The last reported production for this well was submitted in 2015. The tank battery consisted of two 100 BBL production tanks, one 50 BBL steel brine tank and a mechanical separator. The valves on the separator were open and the chart showed no production. The tank valves were equipped with flat plugs. The vent stack on the production tank was plumbed into a 55 gallon charcoal filter then plumbed up and topped with a spark arrestor. The tank battery was diked and fenced with a locked man door. The proper, legible signage was located at the tank battery and the lease access was stable. No violations observed. |

0772736150 | 2007-07-13 | Drill / Deepen / Reopen | No Violations | On site for surface. Operator has drilled to 407 and is setting 365 of surface. Hole was gassing at the time of surface set. I spoke with supervisor Jay Cheslock, Roy Poulson was allowed to run pipe and cement. Operator ran 300LB of gel ahead of cement and one bag of cotton seed hauls. Operator stopped pumping cement once cotton seed hauls returned to surface 150 Sacks of cement. Hole was static once cement returned. Took three samples of cement from the cement tub, no issues with cement mix, 8BBL of cement returned to surface. Operator is going to wait 8 hours before breaking off. Researched water wells logs in the area: House #7704 Berea 143-178 TD201 #7593 Berea 162-199 Water at 180 #7608 158-195 water at 210, #7632 Berea 160-210 water at 212 #7680 Berea 158-195 water at TD 210 Roy Poulson and I had discussed setting surface at 275. The decision was made to set surface deeper due to the depth of the water wells in the area. Shale gas was hit between 340 and 404 feet. |
1168784388 | 2008-09-22 | Preliminary Restoration | No Violations | Site has been seeded and graded. Vegetation is established. No lease road gate, Called Mike Cutter and reminded him that a gate is needed. Well head is a rabbit well. Tank battery has one 100BBL and one 50 BBL tank in it, mechanical separator. Lease road is stable. Tank battery has ID sign. Gates are locked. Well head, tank battery spacing and roadway spacing is per 1509. |
1184612617 | 2007-07-12 | Drill / Deepen / Reopen | No Violations | Operator actively drilling surface hole. Drill depth 188. Operator was taken drill samples every ten feet. Samples at 170 and 180 were a mix of shale and white sand. No clear show of the Berea. Permit calls for 420 of surface casing to be set. There has been discussion of the depth of surface to be set, I spoke with Roy Poulson, I tolled Roy to set 370 of surface, shortening the casing program by 50 Depth of the Berea at the time of inspection is not known. Pits are in good condition, drainage contained on site and the safety fence is up. |
1185457572 | 2007-07-26 | Preliminary Restoration | No Violations | Pits have been solidified, blow pit is hard and the work pit is stiff.
