0363862417 2015-01-20 Final Restoration No Violations My inspection found the 8 5/8 casing plumbed above ground with a plate on it with the permit number. The well is still currently leaking a small amount of gas through the plugs inside the casing. I attempted to contact the company to get an estimate on when they are going to drill out the plugs and replug the well. My calls have not been answered or returned. I will continue to try to make contact with the company.

0552243704 2013-10-22 UrbanProduction Wells No Violations My inspection found the well to be equipped with a pump jack. The well and tank battery are in good repair and are surrounded by a chain link fence. The tank battery consists of three 210 BBL production tanks, a 50 BBL brine tank, and three mechanical separators. The proper identification is located on the tank battery and the well. The lease road is in stable condition. The lease road and tank battery are being shared by permit numbers 2-1877 and 2-1959.
0667892170 2015-03-06 Final Restoration No Violations I was on location for the remainder of the plugging of this oil and gas well. The well was not showing any signs of gas. Appalacian Well Survey is on location to set a wire line bridge plug inside the 8 5/8 casing. Appalacian Well Survey tagged the surface casing plug at 300. The company then pulled up to 225 and set the wire line bridge plug. Nicks ran 1.5 tubing back into the well and tagged the bridge plug at 225. Nicks then broke circulation and pumped 70 sacks of Class A cement on top of the bridge plug and circulated cement to the surface. The remainder of the 1.5 tubing was removed from the well and the hole was topped off with cement.
0968935877 2015-03-09 Final Restoration No Violations I was on location with Pier the well tender for Allied Oil. The 8 5/8 casing has been cut off and a plate with the permit number has been installed on the casing. The pit has been filled and the location has been rough graded. I advised him of what needs to be completed before final restoration can be passed. No violations found.

Permit and company sign was at the entrance of the site. Safety fence was not up at the time of inspection. Site was frozen, BOP was in place.
1170792552 2007-02-02 Drill / Deepen / Reopen No Violations On site for surface cement. Surface not cemented. Surfaced pipe was plugged with glacial till. Operator is going to have to clean out hole with a 17 bit and set conductor. Rig was sinking into the ground at the time of inspection Operator was considering moving rig off and building up site. Complete permit was not on site, called Randy Hall and requested complete permit to be on site by the end of the day. Operator needs to call 24 hours before pit construction, this was not done with this site. Safety fence was not up, needs installed. Silt fence was in place.

1250414095 2014-08-29 Final Restoration No Violations I was informed by the landowner that the plugged well might be leaking. My inspection found the well casing plumbed above ground and was never cut off and capped. The 8 5/8 casing has water in it and is showing signs of gas escaping through the cement plug. The location still has the pumpjack and fence that needs to be removed. The stone pad for the pumpjack needs removed. The cement plugs in the well will have to be drilled out and replugged to correct the issue. I will contact the company about the issues.

1404398370 2014-07-02 Plug / Plug Back No Violations I was on location to witness the plugging of this oil and gas well. Nicks Well Plugging is the contractor doing the plug job for Allied Oil. Nicks stripped the rods and tubing out of the hole, rigged up, and set the Clinton plug through the 4.5 casing with 25 sacks of cement and 50 pounds of cottonseed hulls. The well pressured up to 1500 psi and was shut-in until the following morning. No violations.
1452669930 2007-09-17 Preliminary Restoration No Violations Met with Dan Horning on site. Dan and I agreed that the site needs bladed and seeded, slopes to the drainage ditches need blended into the area. Lease road was stable and constructed out of durable material.
1458988410 2008-08-13 Preliminary Restoration No Violations On site with Dan Horning and Ed Schultz. Lease road is stable and constructed out of durable material. Site has been graded and seeded, vegetation is established. Positive drainage has been obtained during reclamation. Well is in production. Well had been fenced and gate was looked per HB-278. Lease road gate has not been installed at this time. Called Ed Schultz on 08/14/2008, reminded him that a lease road gate is required and that it needs to be installed. Restoration will be passed once gate is installed.
1754387159 2015-03-05 Final Restoration No Violations I was on location for the replug of this well. Alleghany Well Service was able to remove the rest of the drill string from the well and Nicks Well Plugging was able to get their service rig set up on the well. Nicks tripped 1.5 tubing into the well and tagged the Big Lime plug at 2198. The well was loaded with brine water and was showing signs of gas. Nicks pulled the tubing up to 2158 and circulated brine for an hour. Nicks then pumped 58 sacks (200) of Class A cement on top of the Big Lime Plug. Nicks pulled the 1.5 tubing up to 507, broke circulation and pumped 60 sacks of Class A cement to cover the bottom of the surface casing. The tubing was removed from the well and the well was shut in until 3/6/2015.
1989803320 2015-03-04 Final Restoration No Violations My inspection found Alleghany Well Service has drill down through the bottom of the surface casing plug to 507. Upon tripping the drill string out of the hole the rig broke a hydraulic line. The company will not be able to get the rig repaired until the following morning on 3/5/2015. Once the drill string is removed from the well the rig can be removed from the location and Nicks Well Plugging can move their service rig in and start the plugging process.
