0443431709 2013-07-15 UrbanProduction Wells No Violations My inspection found the well to be equipped with a plunger lift unit. There is a house line plumbed into the 4.5 casing. The well and tank battery are in good repair. The well and tank battery are surrounded by a locked chain link fence and pine trees. The tank battery consists of a 200 BBL production tank, a 25 BBL poly brine tank, and a mechanical separator. The production tank is vented through a 55 gallon charcoal filter and into a spark arrestor. The proper identification is located on the tank battery and the wellhead. The lease road is in stable condition.
1051340376 2017-05-15 UrbanProduction Wells No Violations At the time of this inspection, the well was equipped with a plunger lift assembly and the valves on the wellhead were open. There was a domestic line plumbed off of the 4.5 inch casing, with a meter, drip bottle and two regulators. The well was fenced with a locked man door. Production records for this well were up to date. The tank battery consisted of one 100 BBL production tank, one 25 BBL poly brine tank and a mechanical separator. Valves on the separator were open and the chart dated 5/8/17 showed production. Tank valves were equipped with flat plugs and the production tank vent stack was plumbed into a 55 gallon charcoal filter and topped with a spark arrestor. The tank battery was diked and fenced with a locked man door. Proper, legible signage was located at the well and tank battery and the lease access was stable. No violations observed.
1165522044 2006-12-07 Drill / Deepen / Reopen No Violations Pit fluid level is high, 4 inches of free board north site of the pit. Drill fluid and Diesel fuel ponding under drill rig and dog house. Fluids need cleaned up. Operator is going to use absorbent material to clean up fuel. It was requested of the operator to pump as much drill fluid as possible from under the rig and dog house into the pits. Operator stated that he will clean up area into pits once rig is offsite. This is not acceptable, issues such as spills and ponding drill fluids outside of the pits need to be cleaned up immediately. Operator stated that the drill fluids between the rig and dog house came from pulling the drill pipe. It doesnt matter where the fluids came from they need to be contained. Operator had twisted off one section of drill pipe and bit.

1715745899 2007-07-26 Preliminary Restoration No Violations Site has been graded and seeded. Lease road has been restored. Tank battery has been fenced, charcoal filter has been installed. No fencing on the well head. Lightning arrestors on tank battery. Mechanical separator, one 20 BBL plastic tank and one 150BBL tank. Restoration will be passed once vegetation is better established. Site has been seeded and mulched area has less than 1% vegetation on it.
2070142219 2006-11-30 Drill / Deepen / Reopen No Violations Pit linear has slipped on the south east drill pit. Operator is bring equipment on site to repair. Site construction crew was on site at the time, waiting for equipment. North west pit had overflowed and was running under the drill rig. Operator had water truck on site pumping water from the pits. A mixture of drill fluid and motor fluids is under the rig, this will need to be cleaned up. Safety fence was not up. Operator was requested on 11/24/06 to make sure safety fence was up before drilling started back up, this was not done. Construction crew said, they will have it up today. Im not writing NOVs at this time, I called Dave Hill from Alliance informed him of the issues. I will be giving them till this afternoon to correct problems and clean up site.
