0174533666 2017-04-04 UrbanProduction Wells No Violations I was onsite to conduct a reinspection of this well site regarding the replacement of a brine tank. The well was equipped with a plunger lift assembly and the valves on the wellhead were open. There was a domestic line off of the 4.5 inch casing with a meter, drip bottle and two regulators. The well was fenced. The tank battery consisted of one 100 BBL production tank, one 25 BBL poly brine tank, a mechanical separator and sales line. The 25 BBL poly brine tank has been repaired. Valves on the separator were open and the chart showed production. The tank valve was equipped with a flat plug and locked. The tank battery was diked and fenced with a locked man door. Proper, legible signage was located at the well and tank battery. Lease access was stable. No violations observed.
0343067607 2006-10-04 Preliminary Restoration No Violations Site has been graded, mulched and seeded. Pit linear sections were still on site, parts of the silt fence were on site. Dave Hill was contacted, once pit linear and silt fence sections are removed, preliminary restoration will be passed.

1120904884 2017-02-28 UrbanProduction Wells No Violations At the time of this inspection, the well was equipped with a plunger lift assembly and the valves on the wellhead were open. There was a domestic line off of the 4.5 inch casing with a meter, drip bottle and two regulators. The well was fenced. Last reported production for this well was in 2014. The tank battery consisted of one 100 BBL production tank, one 25 BBL poly brine tank, a mechanical separator and sales line. The 25 BBL poly brine tank was crushed and needed to be removed. Valves on the separator were open and the chart dated 2/27/17 showed production. The tank valve was equipped with a flat plug and locked. The tank battery was diked and fenced with a locked man door. Proper, legible signage was located at the well and tank battery. Lease access was stable. I contacted Matt Postlathwaite of Alliance Petroleum about removing the crushed brine tank and adding a lock at the well man door.
1160660886 2006-10-12 Preliminary Restoration No Violations Pit linear pieces still on site. Silt fence and safety fence pieces still on site. Restoration cannot be passed until area is cleaned up of debris.
1184174391 2007-07-10 Preliminary Restoration No Violations On site to meet with Ted from TK Gas. Operator has received complaints from the land owner that the well was drilled on, that reclamation was not sufficient. Site has been graded level. A rock lined drainage channel has been constructed on the west side of the site. Tank battery is fenced. Well head is fenced. Locked gate has been installed. Vegetation is established and has went to seed. I suggested to Ted that the company brush hog the site. Lease road is stable. Pine trees have been planted around the well head and tank battery. One pine tree has died and will need to be replaced. Pit liner and safety fence has been cleaned up around the site.
1373905687 2013-07-15 UrbanProduction Wells No Violations My inspection found the well to be equipped with a plunger lift unit. The annulus valve is closed. There is a house line plumbed into the 4.5 casing. The well and tank battery are in good repair and are surrounded by a locked chain link fence and pine trees. The tank battery consists of a 100 BBL production tank, a 25 BBL poly brine tank, and a mechanical separator. The proper identification is located on the tank battery and the wellhead. The lease road is in stable condition.
