The well and tank battery are in good repair and are surrounded by a locked chain link fence. The tank battery consists of a 100 BBL production tank, a 100 BBL brine tank, and a mechanical separator. The proper identification is located on the tank battery. The lease road is in stable condition and is secured by a locked gate.
0585496047 2018-01-10 UrbanProduction Wells No Violations I conducted an order follow- up inspection on this well to see if the well was in production. The well was equipped with a plunger lift assembly and the annulus and flow line valves were open. There was a domestic line with a meter and a regulator off of the 4.5 inch casing. The valve on the domestic line was open. The well was fenced. Production records for this well were current. The tank battery consisted of one 100 BBL production tank, one 100 BBL brine tank and a mechanical separator. Valves on the separator were open. The tank battery was diked with legible signage. The lease access was stable. I contacted Swallen, Lawhun and Co. who is the firm that does the accounting on behalf of Grimsby Exploration, Inc. and spoke with Kristie Lear, regarding liability insurance. Ms. Lear forwarded a copy of the liability insurance to me, which I forwarded to DOGRM, Surety and Bonding section.

The tank battery was diked with legible signage. The lease access was stable. No violations observed.
