0120397623 2017-10-23 UrbanProduction Wells No Violations During this inspection, the well was equipped with a plunger lift assembly and the annulus and flow line valves were open. The well was fenced with a locked man door. Production records for this well were up to date. The tank battery consisted of one 100 BBL production tank, one 25 BBL poly brine tank and a mechanical separator. Valves on the separator were open and the chart on the sales line dated 10/10/17 showed production. The sales line has been reinstalled and trenched through the property, buried and is no longer exposed within the stream bank. The tank battery was diked and fenced with a locked man door. Signage was located at the tank battery and the lease access was stable. No violations observed.
0724716684 2012-04-23 UrbanProduction Wells No Violations I was on site today to check the stability of the stream bank south of the lease road and the tank battery. The well site is on a Chagrin River meander. The river makes a 90 degree turn against the concave bank on which the lease road is constructed. My inspection found that the Chagrin River has continued to cut into the bank, and the trees the operator had placed along the bank last year, with the approval of the US Army Corps, have been washed away. The top of the bank, where the lease road is located, is 15 above the river and the edge of the bank is 2 to 3 south of the edge of the lease road. The banks lithology from bottom to top is: Shale, 12 of clay, 2.5 of sand and gravel and .5 of topsoil. The sand and gravel has washed out from under the topsoil approximately 3- 4. The area of the washout extends under the lease road which is not safe for truck traffic at this time. The south east corner of the tank battery has also been washed out. A tree has fallen, exposing the sales line and the cemented corner post of the tank battery fence is exposed. I contacted Bruce Close from Alliance Petroleum who stated that he is aware of the problem. As discussed last year, the cost for stream stabilization is prohibitive for this well. Bruce is going to advise his superiors that the well should be plugged. I advised Bruce that the bank stabilization issue will need to be addressed by the land owner in the very near future as the edge of the stream bank is only 30 from the Hruska home. I contacted Chantelle Carroll from the US Army Corp of Engineers for advice. Chantelle has been assisting Alliance Petroleum and the DOGRM with this project. Chantelle suggested the company should get two more options from engineering companies with stream stabilization experience. The issue could be resolved with energy dissipation techniques, rather than rebuilding the stream channel. This information will be forwarded to Bruce. Pictures of the site are on file.
1071671778 2013-06-06 UrbanProduction Wells No Violations My inspection found the well to be equipped with a plunger lift unit. The tank battery and well are in good repair and surrounded by a chain link fence. The tank battery consists of a 100 BBL production tank, a 25 BBL poly brine tank, and a mechanical separator. The proper identification is located on the tank battery. The lease road is in stable condition.
1194365381 2007-11-06 Production Wells No Violations The edge of the lease road on this site had been washed out during floods of last year. Operator has moved the lease road to the north and constructed a berm along the south side of the lease road. Lease road is stable. Tank battery is stable. Meter run is exposed along the bank, the meter run is 8 feet above the river. Well head is a rabbit well. Tank battery has on 210 tank and a 15 bbl plastic tank. Separator in the tank battery is mechanical.
1202922582 2008-02-13 Administrative Inspection No Violations Called Bruce close informed him that Chester Zoning contacted me regarding plastic tank in tank battery. Tank has a hole on the top of it, where it looks like something hit it. Per zoning no leaks. Will do a follow up inspection.
1305035651 2011-05-10 Production Wells No Violations 1500 Gallon poly tank has been removed. Man door has been locked. Load line needs bull plug. Sales line adjacent to the river has not been moved at this time. Contacted Bruce Close from Alliance Petroleum, Bruce stated nothing has been scheduled at this time, to move the line. There was straining on the ground above the domestic supply. Bruce stated that line had broke during the winter and land owner repaired. Bruce will have Gary the pumper clean up stained soil. Stream bank above the domestic supply is unstable from under cutting. Advised Bruce that any type of release into the stream would result in a minimum fine of $10,000 from EPA, additional civil penalties from the DMRM. Gave Bruce the phone number to the US Army Corp in Orwell Ohio, in regards to a Nation Wide permit for bank stabilization.
1321362257 2011-11-14 UrbanProduction Wells No Violations The sales line has been moved to the north side of the lease road. Trees in the stream have been removed with the approval of US Army Corp. of Engineers. A section of the stream bank at the south east corner of the tank battery has slipped. This area has been recently disturbed by the removal of the sales line. The slip will need to be monitored. The wellhead is in good repair. The tank battery is in compliance. Identification sign are posted and correct.
1391202643 2011-04-08 Production Wells No Violations Site is next to a stream, stream has been cutting the bank adjacent to the tank battery and lease road. Sales line is exposed from bank erosion. Domestic meter is supported and suspended by a 2x4 board driven into the bank. Tank battery man door was open. A 1500 gallon poly tank was outside of the battery. Poly tank and 210Bbl tank in the battery. Hatches were closed on the tanks. Separator had a small amount of oil on the ground in the leaf litter. Contacted company requested that the sales line and meter be moved. Tank battery man door be repaired and be able to be locked. Oil to be cleaned up and fittings tightened. Poly tank is scheduled to be removed, next week.
1610876120 2014-08-13 UrbanProduction Wells No Violations My inspection found the sales line has been exposed from river bank erosion. The sales line and the tank battery have been previously moved due to the same issue. No other changes to the location since my last inspection. I will continue to monitor the erosion issue with the location.
