0085298934 2013-12-09 Final Restoration No Violations My inspection found the production equipment has been removed. The fence around the tank battery has been removed. The location has been graded. The location will be seeded and strawed in the spring.
1375276389 2013-07-31 Production Wells Violations Noted My inspection found the well to be equipped with a plunger lift unit. The wellhead is surrounded by a chain link fence. The tank battery consists of a 210 BBL production tank. The separator has been removed from the location. The chain link fence around the tank battery has a section of fence removed from it. Both the tank battery and the wellhead are over grown. There is no identification on the location. I verified the well by GPS coordinates and through the landowner. The well has not shown production since 1999. Petrox was contacted in 2011 by another inspector about the issues. I spoke with Scott from Petrox about the issues. The well must have an identification sign up within seven days of this notice. The well must be produced within 90 days or plugged within a 180 days of this notice.
1383308436 2013-10-23 Plug / Plug Back No Violations I was on location to witness the plugging of this oil and gas well. Nicks Well Plugging is the contractor doing the plug job for Petrox. Nicks stripped the tubing out of the hole, rigged up, and set the Clinton plug through the 4.5 casing with 25 sacks of cement and 50 pounds of cottonseed hulls. The well pressured up to 1500 psi and was shut-in for one hour. Nicks proceeded to rip the casing at 2835. Nicks then pulled 8 joints of 4.5 casing and shut the well in overnight. No violations.
1407420304 2014-08-07 Final Restoration No Violations My inspection found vegetation has been established on the location. All the production equipment has been removed from the location. There is no evidence of the well. The lease road was installed for agricultural use before the well was installed. The flow line from the well to the tank battery was removed. Final restoration can be passed.
1429183738 2011-06-09 Production Wells No Violations Tank battery is overtaken with vegetation, fence is partially removed. Tank battery contains 1-210 BBL tank, no seperator. No ID on tank battery or wellhead. Landowner confirmed well ID as Fath #1 and I confirmed using RBDMS and GPS. Wellhead is shut in. Landowner inquired about status of well, stated he had not seen any activity at the site in a long time. Will contact Petrox regarding status of this well.
1644200443 2013-10-24 Plug / Plug Back No Violations I was on location to witness the remainder of this well plugging job. Nicks rigged up, broke circulation, and pumped 86 sacks of Class A cement to cover the top of the Big Lime. Nicks pulled 50 joints of 4.5 casing, broke circulation, and pumped 134 sacks of Class A cement to cover from the bottom of the surface casing to the surface. Cement was circulated to the surface. The remaining 10 joints of 4.5 casing was removed from the well and the hole was topped off with cement. A total of 68 joints of 4.5, 10.5lb casing was recovered during the job. GPS coordinates are +/- 8.
