1306266253 2011-05-24 Salt Water Injection Well Violations Noted This injection well was on vacuum and not in operation. The annulus pressure was 740 PSI. There was 3 of standing water in the tank battery and there was standing water, with oil floating on top, in the loading pad. It was apparent, by the tracks in the soil, that trucks had been backing up to the battery north of the loading pad. There is an oil and brine scald 10 wide by 60 long running north/ south adjacent to the tank battery. Free oil and brine was also found adjacent to the battery out side of the dike and is currently 20 south of the wetlands. The volume of oil released is estimated at 1 gallon, the brine at 5 to 10 gallons. I called Bob from big Sky and a left message in regards to oil spill and requested a call back. There is a dead tree 3 south of the lease road gate which is cracked 15 off of the ground. This tree will damage the electrical lines if it falls. I have contacted the well owner in regards to the tree and needed removal during previous inspections. Permit identifications signs have been knocked off of the lease road gate. The following corrective actions need taken: 1. Spilled oil and brine needs stabilized to prevent spread of leak. 2. All impacted soil needs removed from location. 3. Peat or another form of bio-remediation needs to be applied to the impacted areas. 4. The water needs to be removed from the loading pad and tank containment area. 5. Broken load line in the pad needs removed or repaired. 6. Open pipe that the load line broke off of, needs a valve or cap put on it. 7. Poly tank in the loading pad needs cleaned out and replaced (back of tank is broken and has 1.5 of oil/debris/sludge in it). 8. The fence around the wellhead needs repaired and a mandoor installed, which must have a lock on it. 9. The lease road gate needs a chain and lock. 10. Identification signs need replaced.
1307474723 2011-06-06 Salt Water Injection Well No Violations No change on site. Identification signs are posted on the gate. Loading pad is flooded. Broken Poly tank with oil and trash debris in it, is still on site. Load line that hose broke off of, no cap on end of pipe. Brine and oil scald is still evident. 12 to 6 inches of water was standing in the dike Fence around the well head is still broken. Valve on the storage tank east side of battery does not have a bull plug. Transfer lines are secured with hose clamps. Electrical boxes are open exposing wiring and breaker. 1309185406 2011-06-24 Salt Water Injection Well No Violations On site with Company representative. Reviewed NOV with Jim. Discussed work that needs to be completed and the time frame he expects to complete work in. Advised Jim that a plan to keep the dike free of water is needed. The current state of the well site is not acceptable. Steps need to be taken to prevent brine haulers from backing up to the dike off of the pad designed for that purpose then spilling brine and oil that remains in the load lines. Pad must be cleaned, entrance of the pad should have rock placed in front of it to allow for better access. Front lip of the off loading pad has approximately a 6 inch gap between ground level and the top of the cement. The lip on the loading pad makes backing into the pad difficult. Means of emptying the sump in the loading pad needs to be maintained.
1311858784 2011-07-27 Salt Water Injection Well No Violations Operator has graded the lease road. Lease road gate has a lock on it. Fence around the well head has been repaired. Tree south of the gate broke and fell during a recent wind storm. Tree took the power line down. Operator has safety tape up around the area. Operator has removed poly tank from the loading pad. Operator cleaned loading pad of debris and water. Debris cleaned from the loading pad is stock piled at the corner of the pad approximately 2 cu ft of material needs to be removed. Water has been removed from the tank battery dike. Cap has been installed on the pipe that the load line broke off of. Load line stand has been placed in the loading pad area to support load lines. Lock has been placed on man door to the well. Oil and brine spill has been remediated. Well head and tank battery still needs to have vegetation cut and removed. Operator improved drainage south of the loading pad. Sump in the loading pad has been cleaned of debris. Well was not operating
1311873310 2011-07-20 Salt Water Injection Well No Violations On site to meet with Jim, employee for Big Sky Energy. Jim was on site with an excavator cleaning up oil and brine scald and improving drainage on site. Dike is free of water at the time of inspection. Fence has been repaired around the wellhead. Jim was actively cleaning the loading pad. Lock for lease road and man door to the wellhead was going to be put on by Jim. Dead tree was still present along the lease road. Jim stated there was an issue with the land owner in regards to the removal of the tree. Jim has installed a siphon in the loading pad to allow him to pump fluid from the loading pad into storage tanks if needed. Cap had been placed on the broken load line. Vegetation still needs removed from the tank battery. Well was not operating.
1312977823 2011-08-09 Salt Water Injection Well No Violations Oil and brine spill has been remediated. The area has had rain the past two days, there was standing water in the off loading pad and the in the tank battery. Lease road gate has a chain and lock on it. Operator has installed fence panel that has the man door on it for the wellhead. There is not a working lock on the man door at the time of inspection. Lock needs to be installed. Two bull plugs need to be installed on valves inside the tank battery. West side of the battery bottom of the center 210bbl tank, the valve needs a bull plug. East side of the tank battery 210Bbl needs a bull plug on a valve. Debris that has been gathered from the off loading pad and stockpiled on the south east corner of the pad still needs to be removed. Vegetation from around the wellhead and tank battery needs to be maintained. Contacted Jim Bailey in regards to the needed repairs.

1329420209 2012-02-16 Salt Water Injection Well No Violations The operator is on site removing vegetative growth from around the wellhead and the tank battery. The tank battery was reasonably clear of water and vegetation. The man door to the wellhead was locked. The lease road gate has a lock. The pump has been removed from location to be rebuilt and, according to the operator, the electric motor also needs re-built. The electric line to the location has not been repaired. I tested the Cloride levels at the loading pad - zero PPM. I tested Cloride levels at the seep that flows from under the loading pad - 78 PPM. The lease road was stable. I spoke with the operator in regards to the site needing additional stone at the turn around by the tank battery and rebuilding the diversion ditch south of the loading pad. Water flows north towards location not south away from location. Bull plugs have been installed on exposed valves. The bottoms of storage tanks are corroded but are currently water tight. The operator is aware that tanks may need replaced. The loading pad was at capacity with water and ice.
1354341516 2011-12-21 Drill / Deepen / Reopen No Violations On site to meet with Jamie, from Big Sky. I informed him that the order requires that the owner/operator submit a written plan to the Division on how to address the water and vegetation issues at this well. The phragmites (tall grass) around the tank battery needs to be removed. Once these two issues are resolved the order will be in compliance. The lease road was stable and a permit sign was posted. Bull plugs were in place and the man door to the well pad was locked. An motor oil leak from Jamies truck spilled into and stained the water under the truck.

My inspection today found the tank battery with approximately 8 inches of standing water in it. The offloading pad was full of water. The lease gate was locked. The man door to the wellhead was locked. The lease road was stable. The well was not operating today. The tubing was on vacuum and the annulus had 720 psi on it. Operator will be contacted to drain the dike and the offloading pad.

The power line running along the east side of the access road is still lying on the ground.

The offloading pad has a couple inches of water in it. The proper identification is located on the lease road gate and the wellhead. The lease road was stable. The well was not in operation. The tubing was on vacuum and the annulus had 750 psi on it.
1942952549 2012-07-11 Salt Water Injection Well No Violations My inspection today found: the wellhead in good repair, surrounded by phragmites. The wells injection pressures were within specified limits. The loading pad sump was full of rain water and had a covering of decaying vegetation on it. The tank battery floor was dry. The Bald Face Hornet nest on the center tank continues to grow in size. Pumper and owner have been contacted for safety reasons about the hornets after last inspection. No use of well noted since last inspection. Lease road has vegetation starting to become established on it.
