0978380381 2017-08-01 Production Wells No Violations I conducted an inspection of the well site. M&R Investments was on location to continue the clean up of the site. The tank battery impacted material had been removed down to the base soil and refilled with clean gravel. The impacted medium had been removed from the site for disposal. The vegetation around the tank battery that had been oil coated had been removed and taken offsite to be disposed of. Cleanup efforts ongoing. M&R Investments will return the next day to continue removal of the oil covered brush.
1501615978 2017-07-31 Production Wells No Violations I responded to an emergency call from the Middlefield Fire Department regarding a 2 inch sales line rupture at this well location. The well and tank battery are owned by Petrox, Inc, but the sales line within the tank battery is owned by M&R Investments. Upon arrival on site, M&R Investments had been notified and was on location. The well and line had been shut in. The approximate 3 inch rupture in the 2 inch poly sales line had sprayed approximately 1 to 2 BBLS of oil within the tank battery, and adjacent vegetation. There was some pooled oil within the tank battery along the west side. No migration of free product was observed outside of the diked battery. I spoke with Bill Fitzmartin of M&R Investments on site and discussed cleanup which would begin the following day. The sales line from the tank battery location ownership is currently being investigated.
1501778761 2017-08-03 Production Wells No Violations I was onsite to conduct an inspection. M&R Investments has completed the oil coated vegetation removal and had seeded and straw mulched the rest of the bare area on site. The cleanup has been completed and tank battery restored. No violations observed.
1713486669 2012-03-28 Production Wells No Violations My inspection of this well site found the tank battery to be equipped with two 210 barrel production tanks and a mechincal separator. The floor of the tank battery dike has 4 of standing water in it. I tested the this water for chlorides, which registered 282 parts per million (PPM). This water also had free oil floating on it and I used absorbent pads to soak up the oil around the production tanks. By the time I returned from inspecting the well, an equal amount of oil had accumulated in this area. The wind is blowing in the direction of the point of accumulation so the free oil is either from the dike floor or a pinhole leak in the tank. I estimate approximately two gallons of free oil in the diked area. I contacted Scott (330- 653-5526) and informed him that the water, oil, pads and empty peat bags need to be removed from the tank battery and the tanks needed inspected.
1970277464 2017-08-02 Production Wells No Violations I was on site to conduct an inspection of the ongoing cleanup activities at the well site. M&R Investments was onsite along with Circle M Excavating conducting the removal of the oil covered brush and small trees. The vegetation is being hauled off site to be disposed of. As the vegetation is being removed the site is being seeded with grass seed and straw mulched.
