1312459435 | 2011-08-03 | Production Wells | No Violations | On site for oil leak at the separator. At approximately 16:00 the resident was mowing the area around the separator. The flow line coming into the separator from the well was hit with the mower. The hit caused a 2 inch lateral cut in the side of the flow line. The release of pressure from the flow lined threw the mower 20 feet west. No injuries to operator of the mower reported. At 16:45 Parkman Fire Department was on scene and shut well down at the demco valve. Pumper on scene shortly after 16:45, jumper adjusted demco valve to completely shut well off. Operator contacted me 17:27 in regards to release. I was on scene at 18:00. Pumper had placed boom in the drainage swale west of the release to contain oil. Pumper had constructed a temporary dam west of the boom as a secondary containment. Oil release was stabilized at this time. The swale runs east and west adjacent to a pasture. The swale has minimal fall, sump and dam will be constructed in the swale to gather residual oil. 19:30 large containment dam had been built by contractor, additional booms were in place. Grossly contaminated soil had been gathered and stockpiled on site. Sprayed vegetation had been cut and stockpiled. Well was repaired at 20:45. Operator will: 1. Remove grossly contaminated soil from location. 2. Remove cut vegetation from location. 3. Maintain sump to gather free crude oil. 4. Apply peat sorb and mix into soil of minimally contamination to add in the break down of the crude oil. 5. Wash down stone lease road with fresh water into sump to remove free crude from the stone. If free crude remains, remove stone from location. 6. During clean up, the removal of the cement brine vault in the tank battery was requested. Vault is no longer in use. The vault was open and flooded with fresh water. Residual oil was coming out of the tank. 7. Remove from site any contaminated soil from around or under the vault |

The tank battery consists of a 100 BBL production tank and a mechanical separator. The tank battery was in need of a dike and there was no identification sign. I spoke with Joe Phillips from Enervest about getting the well repaired. The lease road is in stable condition. |
