0614857896 2014-11-20 Production Wells No Violations My inspection found the well to be 2 7/8 casing plumbed up and reduced to the 2 flow line. The well and tank battery are in good repair. The tank battery is surrounded by a live stock fence. The tank battery consists of a two 100 BBL production tanks, a 25 BBL poly brine tank, and a mechanical separator. The proper identification is located on the tank battery. The lease road is in stable condition.
0824400145 2010-05-28 Production Wells Violations Noted Received call from Burton Fire Department, oil spill at well site. Arrived on site, Jeff Brooks and Ron Botz on site from Circle M Oilfield Transportation (440) 293-6108. They were on site cleaning up spill. This is there 2nd day of clean up. Operator did not contact DMRM or OhioEPA. Per operator valve on the bottom of the tank was broken by farmers horse. There was 1.5 feet of fluid in the bottom of the tank. Oil had escaped the dike and affected an area approximately 18,600 sq feet. Operator had been scrapping and mixing oil into the soil. Operator during the clean up effort filled the dike with fresh water to float oil out of the stone floor of the battery. Employees of Circle M cut dike allowing water and oil mix to flow over field. Per the employees they tasted the water and it tasted fresh before cutting dike. The water remaining in the dike had oil floating on it, operator had spread peat on the fluid, oil was still evident. Test CL- of fluid in the dike CL- was 302. Per the operator spill was 5 barrels. Spill encountered a drain tile and flowed 1200 feet under ground in the tile north east into a stream that is a tributary to Swine Creek. Spill traveled approximately 900 feet north east in the stream. Operator had constructed a sump at the end of the tile and place absorbent pads. The Burton Fire Department placed 8 booms and 1 bag of pads in the stream. EPA had not been contacted by the company regarding the release. I called John Stiker and Danny Glen from Oxford and suggested they contact the EPA immediately. Burton Fire had already contact the EPA. EPA arrived on site. DMRM and EPA agreed Oxford will hand clean oil from the stream, bring water truck on site, pump sump, remove all contaminated soil from the location, strip tank battery and remove all contaminated soil. Contaminated soil will be taken to an approved disposal facility. Land Owner was Paul E. Mast (440) 632-1101, 16950 Shed Road. Danny Glen was given notice of NOV
