At the time of the inspection the final restoration has not been completed and is due by 6-5-14. The cellar, wood/stone pad, and some anchors for the rig still remain around the well. According to the operator the water well on location will be left at the request of the landowner and a landowner waiver will need signed.

0308699694 2013-04-11 Drill / Deepen / Reopen No Violations At the time of the inspection the hole had been re-drilled with the same 14.5 bit along with a 14.75 reamer. The 11.75 casing was ran to 285 and cemented back to surface by Central Michigan Cementing Services. The casing was reciprocated during the cementing. Good cement returns were observed at the surface and 9 bbls of cement returns were noted by the cementers.
0635060652 2013-12-04 Plug / Plug Back No Violations At the time of the inspection plugging operations had started on the well. McS Inc. of Homer, MI and DownHole Tool Inc (DTI) of Traverse City, MI were doing the plugging under the coordination of Matt Fagerstrom (DTI) and Savoy Energy. A cement retainer was set at 2300 and 50 sacks of 50/50 PozMix was set below the retainer and 50 sacks of 50/50 PozMix was set above the retainer. All cement weighed between 14.1 and 14.3 as weighed with cement scales. The well was then shut in to be pressure tested in the morning. All cementing was done by Central Michigan Cementing Services through 2 7/8 tubing with good circulation. See Plug Report for details.
0772473953 2013-12-16 Plug / Plug Back No Violations At the time of the inspection the well had been cut off below grade, the metal plate was welded on, and the hole was filled and graded. A set of production tanks, heater/seperator, pipe trailer and roll-off containter still remain. The mounded topsoil needs graded when weather permits.

The crew was attempting to run the 11.75 casing but could not get the casing past 200. At 2am on 4-11-13, after two hours of trying to run casing past 200, the decision was made to re-drill the hole with the same 14.5 bit along with a 14.75 reamer.

1297455538 2014-09-15 Final Restoration No Violations This well was plugged on 12-4-2013 by McS Inc. for Savoy Energy. At the time of my inspection the gate at the access lane, all storage tanks, separator, and heater unit had all been removed. The stone access lane, dikes, water wells, and some piled topsoil all remain and need removed. The dikes for the tank batteries had all been cut open to drain any fresh water. I spoke with Jack Rokus with Savoy Energy who indicated that they are drilling another well in that area in the next couple months. They plan on using the remaining stone at the new location if necessary and will be using the water wells for water. All remaining restoration work will be completed at that time. Mr. Rokus has spoken with the landowner, Mr. Kunkle, and made him aware of the reason for the delay in the restoration. Final Restoration not passed.

1377794658 2013-08-29 Production Wells No Violations Inspected the Kunkle #1-29 (#125-01) as part of a standard productive well status check. The well is pumping by a propane powered American pump jack and the well pad is clean and free of excess contamination. The storage consist of two tanks set in a lined walled dike and a separate horizontal separator set in a lined walled dike. A 20 flare stack sits north of the separator and is operated by a pilot light (non-operational during inspection). The access road is gated and locked and the well information and emergency contact information is posted at the gate. No trespassing signs and H2S warning signs are posted at the entrance. Status: ok.
1386250259 2013-12-04 Plug / Plug Back No Violations At the time of the inspection the production rods had been removed, a blow out preventer installed, and the production tubing removed. A Weatherford 5.5 Mechanical set Cement Retainer was set at 2300 then pressure tested to 500 psi. Central Michigan Cementing Services pumped 50 sacks of 50/50 PozMix cement below the retainer and 50 sacks of 50/50 PozMix cement above the retainer. Fluids from the well were circulated through the flow line to the Tank Battery. McS Inc. (Homer, MI) was the plugging contractor and coordinated by Matt Fagerstrom from Oilfield Tools, Inc. (Traverse City, MI).
1392155824 2014-02-11 Preliminary Restoration No Violations This well was re-permitted and plugged back for purposes of drilling horizontally. The horizontal leg was permitted as 34-051-20125-01-00 and drilled. The well was then plugged in 12/2013. Preliminary Restoration Passed
1473845713 2013-05-05 Casing String Witnessed No Violations At the time of the inspection a Total Measured Depth of 4361 and a Total Vertical Depth (TVD) of 2433 was reached. Then 2503 of 5.5 casing was ran to a TVD of 2413 and cemented in with 125 sacks of Type I cement by Central Michigan Cementing Services. Good circulation was maintained throughout the job and the water level in the annulus remained static after pumping.
1489406760 2013-04-19 Plug / Plug Back No Violations At the time of the inspection the well was plugged from 2600 to 2200 with 150 sacks of Type 1 Cement with 2% Calcium. This plug was left to set up for 3 hours before the next plug was set. The next plug was set from 2150 to 1700 with 165 sacks of Type 1 Cement with 3% Calcium. The cement was pumped through the drill string and good circulation was observed throughout.
1507225295 2017-10-05 Final Restoration No Violations This well was originally drilled as a vertical well. It was plugged back to a depth of approximately 1700 feet deep and a new permit was issued to kick off this plug and drill horizontally from this well. A new permit was issued, 34-051-2-0125-0100 to drill a horizontal well from this well casing. The new permitted well was eventually drilled and then plugged back to surface. Water wells used for drilling were left and also a stone lease road. A landowner waiver was obtained in order to complete the final restoration. Landowner waiver is on file in the Central Office in Columbus.
1526690916 2017-10-05 Final Restoration No Violations This well was permitted as a horizontal well after the original permit 34-051-2-0125-00-00 was plugged to a depth of approximately 1700 feet. This permit was eventually drilled and plugged back to surface. Water wells used for drilling and a lease road was left after final grading of this site. Landowner waiver is on file in the Central office in Columbus.
1566331585 2013-05-13 Drill / Deepen / Reopen No Violations At the time of the inspection, McS Inc was on location running 2 7/8 tubing in the well to drill out the remaining cement from the cementing of the 5.5 casing with a power swivel. All drilling equipment from Consolidated Drilling has been moved off location. The pit was still open for use in the completion stages of the well.
1572115991 2013-04-14 Drill / Deepen / Reopen No Violations At the time of the inspection 8.625 casing was cemented from 1575 to the surface by Central Michigan Cementing Services. Baskets were placed on joints 2 and 6 from the top and centralizers on the bottom six joints. The casing (All 24 lb/ft except top joint is 32 lb/ft) was reciprocated during the cementing. Good cement returns were observed at the surface and 11 bbls of cement returns were noted by the cementers.
1652707449 2013-12-05 Plug / Plug Back No Violations At the time of the inspection plugging operations were being completed on the well. McS Inc. of Homer, MI and DownHole Tool Inc (DTI) of Traverse City, MI were doing the plugging under the coordination of Matt Fagerstrom (DTI) and Savoy Energy. The 5.5 casing was pressure tested to 500 psi to ensure the plug set on 12-4-13 remained at the proper level. There was no loss noted after 15 minutes. The plug was tagged at 1808 and the 5.5 casing was shot off at 1770, then removed. The tubing was ran to 1750 and plugged from 1750 to the surface with 531 sacks of Class A cement in three stages. Good circulation was maintained throughout and cement returns were witnessed at the surface. See Plug Report for details.
1920535955 2013-04-18 Plug / Plug Back No Violations At the time of the inspection the well was plugged from a Total Depth of 3075 to 2700 with 145 sacks of Type 1 Cement. The cement was pumped through the drill string and good circulation was observed throughout.
