0098257001 2011-03-31 Administrative Inspection No Violations At 8:06 am, I notified Mr. Barry Lay with Envervest Operating LLC, in regards to well status and complaint received. Mr. Lay was in a meeting at the time of my call, but stated that he would retrieve well information and return call. Mr. Lay would return my call and leave a message at 10:30 am. Mr. Lay states that he sent well information to their geologist, as to why they never completed this well. Further stating that he expects it was just a marginal well and probably needs plugged.
0673574794 2011-04-05 Production Wells No Violations Contacted Mr. Barry Lay at 8:44 am by telephone to discuss a timeframe to achieve compliance. A voice mail message was left. Mr. Lay would return call and state that he heard back from Geology Dept and said well was plugged back to base of Clinton formation and currently going through internal review then plugging. Mr. Lay states that Enervest has 80 wells to plug this year in the state of Ohio and 7 have already been plugged. They will have 2 rigs running to meet this goal. Asked what is a reasonable timeframe that Enervest could meet, Mr. Lay responded by stating when the weather breaks, a late summer timeframe could be met. Mr. Lay would further explain they have 10 H2S wells up North that ranks higher in priority. Mr. Lay informed me that he would add me to his weekly report. Left Mr. Ferbrache with Fairfield County Soil and Water a voicemail message with update/timeframe.

1141222230 2006-02-21 Production Wells No Violations Inspection revealed the well is in operation. The drilling pits are closed. No violations noted this date.
1303499528 2011-03-29 Production Wells Violations Noted The Division received a complaint from Mr. Jonathan Ferbrache with the Fairfield County Soil & Water District, alleging idle well. Mr. Ferbrache notified the Division on behalf of the landowner. An inspection conducted with the landowner revealed idle well and no identification. The well consisted of 8.63 casing with a swage and valve. Records indicate a spud date of 9/18/2005 and plug-back to Clinton from Trempealeau formation. Furthermore, the well contains zero reported production and has never been completed. No house gas, flowline or tank battery exist on the premises. Located along the woodline, 7 joints of 4.5 casing and 1 joint of 2 were observed. Photographs were obtained from this inspection. The landowner requests plugging and abandonment, or another operator to place said well back into production. Notified Mr. Ferbrache with Soil & Water by telephone. A message was left at 11:20 am. Mr. Barry Lay with Enervest was also notified of complaint and inspection results. Mr. Lay agreed to plug and abandon this well at end of summer.
