1060884210 2016-05-05 Orphan No Violations No significant changes since Inspector Tompkins report from 9/2014. There was no odor of gas noted in the area, despite calm winds. Area remains paved over, with no signs of any kind indicating the existence of a well. At time of this report, there are no apparent signs or indications of any extreme hazards.
1413579106 2014-10-17 Orphan No Violations I inspected the location of the former well based on on-going reports of gas smell. The area of the former well is located in the asphalt-paved parking lot of the former church. The well has been paved-over, and the location is inferred from historical records. At the time of this inspection, I did not detect any gas odors or detectable readings of combustible gas on handheld device. Continued periodic surveilance is warranted.
1463505811 2016-04-18 Orphan No Violations Todays inspection was conducted to follow up on original complaint #1998 11 034, the former Annunciation Church Orphan well. The church sold the property to Lumen West Realty in 2011. In 1998 an open borehole with gas and oil was discovered slightly inside of the maintenance garage. The borehole was found 20 below ground level. The building directly west of the well and the garage that was built over the well are gone. The area where the garage was located has been paved with asphalt. A low area in the vicinity of 41.426263/-081.77961 is marked with a yellow circle. Dominion marked this area after finding high gas readings coming through the pavement. This area is at the demarcation of the old paved area and the area paved after the garage was torn down. Prior division photos show this is in the area where well was found slightly inside the garage door in 1998. A gas odor is evident in this area and the gas detector showed 100% LEL/0 ppm H2S directly on top of this spot. A drain way cover is approximately 55NW of this spot, 0% LEL detected. Inspector Lowder was also present during todays inspection and photographed the site. See Inspector Moravys 1998 file and Inspector Tompkins 2014 file.
1533046336 2018-07-31 Orphan No Violations I inspected this well location as a follow-up to Complaint #4794 and on-going evaluations for Orphan Well Program inclusion. At the time of my inspection, the area of the former well was comprised of asphalt-paved parking and drive areas. There was no surface expression of the well, and the location is inferred from previous inspections, photos, and descriptions. There was some gas odor near the suspected location but no meter-detected levels.
