0028284390 2016-10-18 Orphan No Violations I was at this location pursuant to a request from Olmsted Twp. Fire Chief Pat Kelly. Activity at this location consisted of the following: The site owners contractor Rettew was on location to begin implementation of remedial system for mitigation of stray gas. The proposed elements of the system will include a shallow vapor recovery trench and associated replacement of the existing well vault with a larger vault, inter-connected with the recovery trench. The existing water well is to be removed and replaced with a PVC-casing for enhanced vapor recovery. At the time of this inspection, crews were drilling shallow borings which are to be part of the vapor recovery trench system. I met with representatives of Olmsted Farms Ltd., Rettew, and Fire chief Pat Kelley. We discussed the activities and Chief Kelley was satisfied that adequate safety measures were in place. No gas readings in ambient air were being detected.
0140994647 2016-03-30 Orphan No Violations I inspected this well location pursuant to a report of possible gas leak (Complaint #5293). I was contacted by Brian Tomcho with Columbia gas who reported elevated gas readings detected by a leak detection crew. The elevated readings were reported near a well vault structure near an apartment building. Upon my arrival, I met with Columbia crews on location and in the process of performing a field test of the service line. I observed the following: a 4 x 4 concrete vault structure adjacent to the north exterior wall of a multi-tenant apartment building. The top of the vault is flush with grade and has a 30-inch diameter cast iron, bolt-down access lid. There is a vent stack structure located 15 feet north of the vault. The vent stack is comprised of 3-inch solvent-welded PVC material painted green. Columbia crews report intermittent readings of up to 6% total gas in punch bar locations near the vault, between the vault and the exterior wall. Upon my inspection, I did not detect any gas in ambient air near the vault or at the vent. I removed the vault lid and found a significant build-up of gas pressure. The interior of the vault was partially filled with water obscuring much of the interior structure. Gas readings in the vault were 50 to 75% LEL sustained. These findings suggest that the passive vent line from the vault is plugged. I recommended that the vault lid remain off to facilitate venting. Columbia reports no gas detections inside the building. I filed a report with the OEPA emergency response and with the Olmsted Township Fire Department. Columbia reported that the nominal depth of the gas service line is 18 inches. The depth to the bottom of the vault was not discernible, but is at least 3 feet. I met with the building leasing manager and we discussed the findings. Columbia had completed the test of the service line and did not find any problems with the line integrity. I recommended that the vault be permitted to vent, followed by a reevaluation of the p
0293717818 2016-04-12 Orphan No Violations I met with Fire Chief Patrick Kelley, William Gallagher (Building Dept. Rep), Steve Vogel (Building Dept. Rep), Trent Dye (Gross Builders), Chris Sheely (Gross Builders), and Brian Tomcho (Columbia Gas) to discuss on-going evaluation of the well, and plans to mitigate gas leaking from the well. At the time of my inspection, crews from Gross Builders had excavated and exposed the vent stack lateral. Based on an assessment of the lateral, the grading of the 3-inch PVC is sloped away from the vault and causing water to accumulate in the vent. We discussed the need to re-grade the lateral to facilitate drainage as well as possible regrade of the surrounding soils to facilitate local drainage and mitigate infiltration of groundwater into the vault. The FD and Building Department Reps requested submission of a plan to them by Gross Builders of final grading and dewatering engineering plans. Gas was observed to be bubbling up through shallow, accumulated water in the vault. Using hand-held metering, Columbia gas surveyed the area around the well, along the building footer, as well inside the building, and did not detect measurable levels of gas.

At the time of this inspection, efforts were in progress to over-drill the former water well and convert to an extraction point, which is planned to be integral with a vapor extraction system. A sub-surface washout had occurred previously and crews were working to replace the washed-out material with imported gravel.

1459784901 2016-04-01 Orphan No Violations I inspected this well location as a follow-up to a previous report of gas detected near the well vault by Columbia Gas leak detection crews (Complaint #5293). At the time of the initial report and inspection (3/30), gas was noted to be collecting in the vault interior, and not being vented through the existing passive vent structure. The vault was left open to vent during subsequent assessment. Columbia field crews tested and found the service line to be free from leakage. Maintenance crews from the property (Gross Builders) performed an assessment of the vent stack and lateral and found water collected within the lateral. The water was removed and the lid on the vault replaced pending further assessment after the weekend. At the time of this inspection, I did not detect and gas near the well or the vent stack. I contacted Chris with Gross Builders and he reported that their crews had dewatered the vault interior and were going to reassess next week. They had also made contact with Baker Well Services, who had originally constructed the vault and vent system. Rex Baker indicated that he would confer with Gross Builders, would provide an as-built diagram of the vault, and assist with the on-going evaluation. We agreed to meet at the location on a day and time to be determined next week.
1462206645 2016-04-08 Orphan No Violations I inspected the well location as a follow-up to the previous inspection on 04/01. At the time of this inspection, the well vault lid was closed and secure. I met with FD and Gross Builders Representative Chris Sheely. Upon removal of the vault lid we discovered the vault to be filled with water. Gross maintenance crews retrieved a pump and began dewatering the vault interior. I observed gas bubbling from around the wellhead. Gas readings within the vault were observed to be 50 to 60% LEL once the water had been evacuated. Pumping of water from the vent laterals was executed. I recommended to Gross that any further assessment should include removal of the well vent header to facilitate inspection of the well casing top and inspection of the adjacent water well. No detectable gas readings were observed in the interior of the building.
1468248479 2016-06-14 Orphan No Violations I was on location to observe activities being conducted by Rettew and Moore Resources to expose and examine the interior of the existing vault structure and top of the gas and water well casings. At the time of these activities, the interior of the vault was observed to be dry. Rettew and Moore crews removed the existing PVC vent header at the inferred water well and gas well casing tops. Upon inspection, there was cement present within each casing flush with the top of the casings. The cement plugs appeared solid and stable with no signs of degradation or problems. Further inspection using water/detergent mixture indicated no evidence of gas leakage from either casing.
1469046076 2016-07-17 Administrative Inspection No Violations I attended at meeting being conducted by Evergreen and Rettew to present the findings of the recently completed investigation and proposed mitigation for the stray gas problem to the FD, Township trustees and their consultants. At the time of this meeting, the interim remedial venting system remains in place and functioning. Residents remain displaced from the building. The gas detection system has been deployed in the building and has not had any reported alarm conditions. Work will continue to collect data and refine the remedial system until implementation of the final mitigation design and approval of re-occupancy by the FD.
1479138370 2016-11-01 Orphan No Violations I was at this location to witness on-going activities associated with efforts to mitigate stray gas inferred to be leaking from this well. At the time of this inspection, crews from Rettew and Moore Well Service were working to replace the concrete well vault structure which had been placed at the well site at the time of the previous plugging and abandonment activity. A larger vault was being installed as part of the system designed to capture the stray gas. During these activates, the temporary blower system was left in-place to continue to mitigate the gas until the final system is completed. I noted that no elevated gas readings were being detected in areas beyond the vault.

Hall to discuss progress on mitigation associated with a leaking gas well adjaent to the apartment building. At the time of this meeting, efforts were underway to mitigate gas at the wellhead vault and assess whether leaking gas is infiltrating the sub-slab areas. Residents were evacuated on 04/20. No detectable levels of gas have been recorded inside the apartments. Levels of gas up to 100% LEL have been reported in soil punch bar locations near the vault. Rettew Associates are being contracted by Gross Builders to assisst with mitigation engineering design. Partners Environmental has conducted some intitial gas survey work on the exterior and interior.

Gross plans to place several sub-slab gas monitors within the building. At the time of this inspection, the well vault lid was closed and secure. The interim mitigation system is in operation. Gas readings at the outlet of the vent stack did not exceed 2% LEL. No detectable gas readings were observed outside the vault area. Olmsted Twp. Fire Department personnel have been conducting twice-daily surveys of gas inside the building. Chief Kelly reports that no detections of gas have been recorded at any of the interior locations. The reps from Fox & Dole with provide Gross Builders with a proposal to provide monitors and costing for installation. The logistics for notification from the monitors will be worked out later but may include connection to an auto-notification system to the FD.
