0211968646 2017-10-16 Plug / Plug Back No Violations 1 diameter tubing(1 joint) removed from well, 1 diameter tubing head installed. 1 diameter tubing being ran inside 2 tubing to clean out. After running 5 loints (+/- 165 feet,) with no apparent obstructions, decided to disconnect swivel and run tubing. Went to approximately 180 feet and sat down. Was able to rotate tubing a bit further in, but plugged tubing off. Hooked up mud pump to tubing and circukated it clear. Current depth INSIDE TUBING is +/- 300 feet. Started to pressure up upon circulating (500 PSI.) Decision to log hole in morning to ascertain whether or not we are in open hole.

Unable to pull tubing. Retrieved overshot from well and had a collar from the top joint inside. Difficulty getting bck on tubing with overshot. Finally able to get on tubing with overshot. Pulled 15,000 PSI on tubing and it parted. Have removed one joint of tubing from well. Going back in hole with overshot to try to pull more tubing. Waste Management water truck on location to remove water from steel pit. Have gotten back onto tubing but can not pull from hole. Have pulled 40,000 pounds and moved tubing up hole +/-15 feet. Pipe has stopped at this point Ran weighted line down ther anular space to a depth of 110 feet. (Rig crew will not be on location until sometime late Monday as they have a mandatory safety meeting.).

Casing is 6.5 OD diameter with 11V thread. Calls being made to attempt to locate similar pipe. Tarps being placed over security fencing to protect houses. Rig arrived on location. Rig crew returned with welded 7 and installed same on casing. Container with contaminated soil removed.

First joint, 31.62 feet, washed over tubing. Approximately 120 feet in, have come up against what returns indicate is metal. Tripping out of hole to go in with overshot and attempt to pull tubing. Have attempted repeatedly to part the tubing with no success. New plan is to go in with external cutter, inside washover pipe, and cut tubing at a pre-determined depth. At that point, likely will begin milling. We can hear a VERY SLIGHT bubbling inside tubing. On the chance that the tubing is now clear, we will still cut it at a pre-determined depth, install a centralizer and attempt to run 1.5 inch diameter tubing inside.

1441821105 2015-08-14 Orphan No Violations I inspected this well location as an evaluation of current site conditions. The well is located in the front lawn portion of a private residence. The location as reported by the current land owner corresponds to a 10-foot diameter dead spot in the yard. At the time of this inspection, there is no detectable gas in the vicinity of the dead spot. With the exception of the dead spot, the owner does not report any other problems with the well. Access to the area of the well is generally open with no significant obstructions.
1478631848 2016-11-08 Orphan No Violations Went to location this date to expose well and prepare for plugging in the morning. After exposing the well head inside the vault, and releasing the pressure from the well, we removed the 3 bolt head (something that was not done on the previous excavation of this well,) and discovered that there is an unkown amount of 2 tubing that appears to have been fire clayed in. We ran a weighted line down inside the tubing to an unobstructed depth of 15, and ran the same line down the annular space to a depth of only 3. We attempted to pull the tubing with a small excavator to no avail. After installing a 1 pressure relief valve on the side of the casing the well head and vault were covered back over pending a decision on the next course of action.

Plug is down, tubing tripped out of hole, well shut in until morning.
