0613219318 2015-03-02 UrbanProduction Wells No Violations I inspected this well as part of routine inspection activity. The wellhead and battery are located behind the Concordia Lutheran Church on Brecksville Road. Access to the wellhead is from the Concordia Church at Access to the battery is from the Cleveland Clinic Home Care office facility on Brecksville Road. The wellhead is comprised of a mechanical plunger lift system in a board-on-board enclosure. The battery is comprised of 6 100-barrel production tanks, 1 50-barrel brine tank, and 3 separators inside a separate chain-link enclosure. The battery is shared with the #2-1862 and the #2-2080 wells. Fittings and connections at the wellhead are secure and free from signs of leakage. Gates and load-outs are secure and locked. Signage is present and complete. I did not observe any evidence of impact relating to on-going production activity.
1182335139 2017-08-28 UrbanProduction Wells No Violations I inspected this producing well location as part of routine inspection activity. At the time of this inspection, I observed the following: a wellhead with plunger-lift system and associated tank battery, shared with the Concordia #1-D, and the Scharton #1, located behind a church. Fittings and connections at the wellhead and battery were secure. The enclosure gates were secure. The load-lines were secure. The secondary containment was intact and dry. The dedicated access road to the well and battery which enters from the adjacent Cleveland Clinic facility, was in good condition. I found no signs of problems at this inspection.
1212409715 2013-05-20 Drill / Deepen / Reopen No Violations Crews are beginning to make the location for this unit. Based on my discussions with John from Wildcat, we agree to re-orient the pits to the east to alleviate the need to cut trees and do additional grading, site work restoration. Equipment is arriving but pit construction has not started.
1371075158 2013-06-12 Urban Drill / Deepen / Reopen No Violations Crews from Circle C Excavating are onsite closing drilling pits and grading site. I discuss with Ralph Gregory the results of the 5-day annular pressure monitoring. He indicated the annular pressure has been fairly steady at 60 PSI since well completion.
1371217044 2013-06-14 Drill / Deepen / Reopen No Violations Crews from Appalachian Well Surveys and Nabors are here for well stimulation (perforation and acidize). No problems observed or noted.
1423185400 2013-05-25 Drill / Deepen / Reopen No Violations 11-inch conductor set and cemented to surface this morning. I talk to Tom Wood about making sure the cellar is made liquid tight as required. The cellar was then lined with plastic and is holding fluid. The pit liners are in good condition and there is ample freeboard. The crews are shutting down for the remainder of the holiday weekend.
1508096637 2013-05-24 Drill / Deepen / Reopen No Violations Wildcats rig is onsite and rigging up. I talk to John about getting booms set up around the storm drain inlet. Pits are being completed and lined.
1517687873 2013-05-30 Drill / Deepen / Reopen No Violations Drilling is continuing. Cellar and pits are in good shape. No appreciable fluid loses during drilling. No problems reported or observed.
1705115786 2013-05-29 Drill / Deepen / Reopen No Violations Blow-out preventer tested and passed this morning. Continuation of fluid drilling with no problems reported or observed.
1818164938 2013-05-22 Drill / Deepen / Reopen No Violations Site preparation work is continuing. The silt fencing is in place and looks good. Pit construction has not started yet. The site is being graded and topsoil is being stockpiled on the east side.
2009860756 2013-06-01 Drill / Deepen / Reopen No Violations Drilling at 3,600, with a projected TD of 3820 +/-. Will likely reach TD late tonight, and run casing in the morning.
2040492489 2013-09-23 UrbanPreliminary Restoration No Violations The wellhead has been fitted with a mechanical plunger lift system and is enclosed in a board-on-board fence which is locked. The 8 5/8ths casing is plumbed into production and is showing 50# of pressure. The surrounding area including former pits and areas disturbed by drilling had been re-graded and seeded. Some grass is taking root. The aboveground storage tank (AST) battery is being shared with the Concordia 1-D (#1862) and the Scharton 1 (#2080). The battery is comprised of 6 100-barrel steel production tanks and 1 50-barrel steel brine tank. Spacing and safety equipment are in accordance with rules. Gates are locked and ID placard is present. Secondary containment is adequate. No problems or violations. Preliminary restoration passes.
