Entry gates are locked and secure, at both the wellhead and battery. Process safety equipment is present. Fittings and connections are secure and free from signs of leakage or problems. Tank hatches are closed and secure. The short, paved access road is dry, level, and well drained. At the time of this inspection, I observed the following deficiencies: The load line is not lock protected, ID signage is lacking required information. Warning signage is incomplete. These deficiencies must be corrected as soon as practicable or by 06/29/2015. Please contact Inspector Stephen Tompkins at 330-907-4647 for questions regarding this inspection.

1443931463 2012-11-07 UrbanPreliminary Restoration No Violations My visit to the well today was for an Urban Preliminary Inspection. This unit consists of a Linger Rod Vertical pump unit and there is 200 PSI at the annulus. Tank battery consists of two (2) 140 Barrel lowboy tank units with vents, charcoal filter units for each tank. Well site is in good condition and dike around tank unit is capable of holding fluids per 1501:9-9-05. The preliminary restoration is not complete, as site has not been seeded due to considerable rainfall since the well was completed. Discussed situation with maintenance foreman, Larry Gibler. He assured me that as soon as weather permits he would plant seed.

The producing unit is comprised of a wellhead with valve assembly and linear rod pump system. The unit is producing and production reports are current. ID placards are present and have current and accurate contact information. The tank battery is comprised of 2 100-barrel steel production tanks, 1 50-barrel brine tank, and separator unit. There is a Dominion metering run in the enclsoure. The secondary containment dike is is good condittion. The battery lies wtihin a naturally low-lying area and drainage is likely from infiltration. I did not observe any signs of gully formation, wash-outs, or other impacts associated with the unit. Enclosure gates are locked. I did not observe any signs of leakage, spills, or other problem conditions.

Enclosure gates and electrical panels are locked and secure. The battery secondary containment is intact and free from signs of leakage or problems. Spark arrestors, low-pressure relief valves, and carbon filter is present. The tank lids are closed and secure. The 8.625-inch well casing is exposed and open to atmosphere. There is no discernible gas venting from the casing. The previous violations have been addressed.

The wellhead linear rod pump system is located in a board-on-board enclosure in an open lot. The surrounding area is comprised primarily of residential apartments and some single-family homes. The battery is located via a short (250 ft.) dedicated access road which terminates in a wooded area. Access gates at the battery and wellhead are locked and secure. Load-outs at the battery are bull-plugged and secure. Connections and fittings are secure and free from signs of leakage or other problems.
