The unit is producing and production reports are current. ID placards are present and have current and accurate contact information. The tank battery secondary containment dike is is good condittion. The unit lies in a failry level, suburban residential area. Surface runoff is controlled by storm sewer infrastructure. I did not observe any signs of gully formation, wash-outs, or other impacts associated with the unit. Enclosure gates are locked. I did not observe any signs of leakage, spills, or other problem conditions.

At the time of this inspection, I noted the following: a wellhead with pump-jacksystem and associated tank battery in separate board-on-board enclosures located on a residential parcel. The enclosure gates were locked and secure. Fittings and connections at the wellhead were secure. The pump-jack supports were solid. The stuffing box fitting was secure with no significant leaks. The bridle attachments were secure. Fittings and connections at the battery were secure. The load-line valves were plugged and locked. The tank lids were closed and sealed. The secondary containment drain was secure. There was some standing water in the containment area from recent rainfall. The water was clear with no signs of floating oil or sheen. The road is comprised of a shared-use, concrete driveway which was in god condition. I walked the areas surrounding the well and battery systems and observed the natural soils and vegetation to be free from signs of leaks, spills, or other adverse impact from the on-going production activity.

The shared driveway access to the tank and battery are in good condition. The gate is secure and ID signage is in place at the entry point. Enclosure gates, load-lines, and electrical boxes are locked and secure. Fittings and connections at the wellhead are secure. The battery secondary containment is intact, and free from signs of leakage. Areas around the well and battery consist of well-manicured lawn. I did not observe any signs of problems at the time of this inspection. I contacted the emergency number listed to confirm accuracy. The number is functioning and correct.

Entry gates are locked and secure, at both the wellhead and battery. Process safety equipment is present. Fittings and connections are secure and free from signs of leakage or problems. At the time of this inspection, I observed the following deficiencies: the load lines are not lock protected, the electrical box is not lock protected, there is an unprotected extension cord running over ground between the tank battery and the wellhead. There is no evergreen screening which is required as part of site restoration, unless waived by the landowner. ID signage is lacking required information. Warning signage is incomplete. These deficiencies must be corrected as soon as practicable or by 06/29/2015. Please contact Inspector Stephen Tompkins at 330-907-4647 for questions regarding this inspection.

Valves and connections are secure and free from signs of leakage problems. At the time of my inspection, the area is covered with several inches of snow. Access to the wellhead and battery are via the shared residential driveway which is snowed in but passable. Valves and connections at the battery are secure. Load-outs are bull-plugged and locked. I did not observe any problems at the time of this inspection.

Supervisor Tom Hill emailed Rick Little to address water issue inside of tank battery.The tank battery floor was covered with approximately six inches of storm water.

Following maintenance issues must be addressed: 1. Water needs to be drained from tank battery.

1433692917 2012-11-27 UrbanPreliminary Restoration No Violations On my visit to the well today I was accompanied by Steve Tompkins, Lake County Inspector, DOGRM. The purpose of the inspection was to perform an Urban Preliminary Restoration Inspection. The access road is concrete for the first one hundred feet off the roadway. The well head is enclosed by board on board fencing and is secured with chains and locks. There are no violations at the well head. The tank battery is enclosed inside board on board fencing and is locked and secured by locks and chains. There is one 140 BBL storage tank and one 70 BBL brine tank. There is one vertical and one horizontal separator unit in the tank battery. There is a metering run also. Pick up lines are enclosed in the tank battery and secured by locks and bull plugs. The area has been seeded and grass is growing through out the well site. No violations found. Urban Preliminary Inspection passed.
