Access road is concrete for the first 60 feet, then crushed limestone and brick throughout the remainder of the well site. Grass has been seeded and restored to specifications in the pre site report. The well head is encased inside a board on board fence mounted on top of a dike surrounding the well head. The gate into the well head is secured by lock and chain and is secure. No issues or violations are noted. The tank battery is enclosed inside board on board fence on top of the dike surrounding the tank battery. Pine stock is planted on the dike for erosion control. The dike is capable of holding the amount of fluid prescribed in 1501:09-5-5. The tank battery has two 140 BBL storage tanks and 140 BBL brine tank. Caps on storage tanks are closed and secure. There are vent caps in place as well as a charcoal filter unit. There is one vertical and one horizontal separator, one metering run and load lines which are secured by locks and bull plugs. There are no violations noted. Urban Preliminary Restoration passed. |

The well is located behind a single-family dwelling on Royalwood Road. There is a short access road beyond the shared driveway. At the time of this inspection, there are no wash-outs or erosional deficits. The wellhead is comprised of a pump jack unit in a board-on-board enclosure which is locked. The entry door is secure and locked. At the time of this inspection, the following maintenance issues were noted: The permit number indicated on the signage is incorrect. Electrical boxes are not locked. Production tank lids are not latched down as required. Signage is present but lacking complete information. There is some oil leaking at the back-pressure (Baird) valve and at the stuffing box. An empty 55-gallon drum and chemical pump should be removed if not used. Carbon filters must be periodically evaluated for properly operation. In addition, the following safety issues were noted. Load lines are not locked. These safety issues must be remedied as soon as practicable or by 06/19/2015. Please contact Inspector Stephen Tompkins with questions regarding this Compliance Notice. |

Well is currently shut in, and company will not be putting well into service until at least Wednesday, 11/02/2011. Cutter explained that problem arose due to a foreign object in the stem and seat of the dump valve, allowing gas to bypass into the atmosphere. Cutter Oil has been in contact with the gas service company concerning getting the service/sales line in service. Waiting for the response of the Dominion/East Ohio gas company at this time. |

My inspection found a small puddle of oil at the stuffing box with diapers soaking up the oil. Company was notified by phone to make repairs as needed. No violation to be issued. A follow up inspection will be conducted within 30, days. The wellhead is in good operating order and is enclosed in a board on board fence and is secured with locks. There is 200 PSI at the annulus. The tank battery is enclosed in a board on board fence and has two 140 Barrel lowboy tanks with vent caps in place. There is a charcoal filter in place and spark arrestors on the tanks. There is one 70 Barrel lowboy brine tank, metering run and a separator. There are two load lines which are secured with locks. Well is in compliance with 1501:9-9-05. Urban Preliminary restoration not passed until follow up inspection to be performed. |

She is concerned about the number of wells in Broadview Hts, the proximity of this well to her home and also if this well will be fraced when it is completed. I called Deputy Chief Tugend to report that I found no violation and that the well is in compliance.

1320066357 | 2011-10-27 | Production Wells | No Violations | On 10/27/2011 Inspector Ray Wallace received a phone call from DNR Inspector Norburt Lowder at approximately 5:45 PM, that he had received a call from Michele Aini, 3128 Osage Way, Broadview Heights, OH. Complaint was a very strong odor of gas from the vicinity of the Becker well #1, permit # 22193, located at 2779 Royalwood Road, Broadview Heights, OH. Upon further investigation following several phone calls to City Building department, (Roger), and Fire chief, (Jeffrey Hajek), discovered that fire department had been on scene at 3092 Osage Way earlier in the day, (Lisa Sabula), property is located due south of the tank battery. Call to BHFD came in at 3:51 PM, BHFD on scene at 3:53 PM. Cutter oil employees were on site. Fire Department inspected area of tank battery and found an open 2 fitting hole not covered in one of the tanks. Fire department took reading of 2% LEL at tank battery area. Open hole was for stem and seat of dump valve for tanks. Cutter employees placed a plug in the hole, and fire department rechecked LEL and readings were zero. Fire department cleared scene at 4:36 PM. ODNR was not notified by either BHFD or Cutter Oil of this incident. Inspector Wallace contacted Mrs. Aini, who complained about the earlier complaint and stated that there was still an odor in the air. Inspector Wallace attempted to contact CJ Cutter, (well owner), and President of Cutter oil. Mrs. Ainis property is located approximately 600 SSW of the well head. Mr. Cutter contacted DNR Inspector Wallace at about 7:40 PM and informed him that he had company representative Tony Horvath shut the well down, and to remove it from service. Inspector Wallace called Mrs. Aini to inform her that the well had been shut down and would remain that way until Friday morning. DNR Inspector Wallace further informed her that he would not be traveling to the well site, but should any further incidents occur, feel free to call him and he would travel to the well site.
