0027122119 2013-01-03 UrbanProduction Wells No Violations Mitigation activities are on-going following a release from a failure of the stuffing box seal on 12/31/2012. It appears that a majority of the oil released was contained by the dike at the wellhead. A small amount of oil, estimated to be not more than 2 or 3 barrels, escaped the dike and traveled 100 +/- feet down a small drainage channel to the east-northeast. At the time of my visit, most of the oil and impacted soil has been scraped up. An interceptor trench has been constructed down-gradient. Crews plan to place booms at the interceptor trench. I suggested that an additional boom be placed further down gradient as a safeguard. Additionally, I recommended that clay material be added to the dike material to help mitigate any future failures of the dike. The stuffing box is fitted with a leak detection/shut-off device which apparently did not function as designed. Doug Gonzalez indicated that he will investigate the cause of the failure.

The enclosures are secured with locks and in in good condition. The tank battery is comprised of five 140-barrel steel tanks, a 25-barrel poly tank, and mechanical separator. HB 278 safety equipment is in place. Secondary containment is present and adequate. The tank battery is shared with the Moore Well Service Persuric/Kolar 1-D (#2-2182). The downslope area from the pumping unit had formerly been impacted with a release of oil from a failed stuffing box seal. The area has been completely remediated and restored. Nothing further is necessary regarding the oil release. I did not observe any other problems or violations.

1354212675 2012-11-29 UrbanPreliminary Restoration No Violations My visit to the well today was to conduct an Urban Preliminary Restoration inspection. The wellhead is enclosed within a chain link fence with slats. The wellhead area is clean and no violations were discovered. The tank battery is neat and clean and free of debris. The dike is of sufficient capacity to meet the standards of 1501:09-5-5. There are two 140 BBL storage tanks and one 50 BBL brine tank. The storage tank caps are closed and secure and a vent system is in place. There is a charcoal filter system in place. There is one vertical separator and a metering run in the tank battery. The pick up load lines extend through the dike and are locked and have bullhead plugs in place. The access road is concrete for the first 150 feet and the remainder is crushed and packed limestone and the landscaping is complete. The preliminary restoration is complete and passed.
1357329054 2013-01-02 UrbanProduction Wells No Violations I went to assess this unit persuant to a call from Doug Gonzalez of Gonzoil. On 12/31/2012, the pumper noticed that the diked area around the pumping unit at the wellhead had filled partially with oil from a failed stuffing box seal. Some amount of oil had penetrated the dike and traveled down the hill with the runoff water approximatley 50 feet and ponded adjacent to the Persuric/Kolar 1-D Unit. Some floating oil had then traveled about 50 feet further northeast. Gonzoils crews were digging an interceptor trench downgradient of the oil and were beginning to clean the oil from within the diked area. The limits of impact appear to be limited to this area and no oil is evident beyond the landscaped portion of the unit. Mitigation was to continue today, including the placement of absorbent pads and booms within the spill area and downgradient. The plan includes the removal of impacted soil with a mini-excavator and direct load to a dump truck for off-site removal tomorrow.

The well and associated battery are located behind the residence at the referenced address and accessed via a gravel-paved access road. At the time of this inspection, fittings and connections at the wellhead and battery were secure and free from signs of leakage or problems. The load-lines were plugged, locked, and secure. Secondary containment was intact and secure. The individual enclosures were locked and secure. The access road was well drained with no signs of wash-outs or erosional deficits. ID signage was present. I noted that the metal stairs on the interior of the tank battery had become rusted with some broken rungs, creating a potential hazard. I notified Doug Gonzalez about the need for maintenance.

The wellhead pump jack and battery enclosure gates are secure and locked. Load-outs are plugged and secure. The containment interior has a small . Connections and fittings are secure and free from signs of leakage or problems. The gravel-paved lease road is well maintained and passable. Drainage from the lease road and areas surrounding the wellhead and battery is free from signs of impact or erosional deficits.
