The wellhead and tank battery are located behind a single-family residence on Avery Rd. The area is comprised of two other producing units (Kral 2D and Kral 3D). The three wellheads share the common battery. Fittings and connections at the pump jack and battery are secure and free from signs of leakage or problems. The well was in production with active chart record. The interior of the secondary containment was clean and dry. Load-outs were plugged and gates are locked. 0816431782 2012-11-27 UrbanPreliminary Restoration No Violations My visit to the well today was for an Urban Preliminary Restoration inspection. The wellhead location is fenced with chain link with slats and the well head area is neat and clean with no violations noted. The gate is locked and secure. The tank battery location is shared with the Kral 2D and the KRAL 3D wells. There are six (6) tanks in the battery and each well has its own two (2) tank system. The tank battery is surrounded by chain link fence, which is locked and secure. There are two 140 BBL storage tanks. The tank caps are closed and secured and vents are in place and secure. A charcoal filter unit is in place. There are two pickup lines through the dike and they are secured with locks and bull plugs. There is one vertical and one horizontal separator in place and one 70 BBL brine tank. There is a metering run in the unit. There is grass planted and growing and the access road is compacted limestone. Well has no violations and Restoration Inspection is passed.
1006330698 2011-09-24 Urban Drill / Deepen / Reopen No Violations On 9/24/2011 Inspector Norburt Lowder received a call from Wade Balser of OEPA regarding a complaint he received from the Broadview Heights Fire Department (BHFD) of oil bubbling up in the back yard of 8490 Wyatt Road in Broadview heights, Ohio. This property is owned by Irene Krotz and is located directly south of the Kral 1D (#2187). Mr. Lowder then contacted Gene Chini and myself of ODNR, GonZoil and Mrs. Krotz about the problem. I arrived at the site at approximately 5:15 PM on 9/24/2011. I inspected the back yards southeast of the well site, (Krotz, Tremblay and Benes), and discovered evidence of petroleum on the grass in a small depression that ran through the Krotz and Tremblay back yards. I then investigated the Kral 1D drilling site and noticed the soil adjacent to the drilling rig air compressor and mud pumps was covered with PeatZorb. There were several small areas of motor oil on the ground surface which appeared to be flowing into a small pool of water that had collected on the east side of the location. I also found some broken clay field tile adjacent to this pool of water, and upon further investigation, the opening to solid clay tile which appeared to run south along the east edge of the drilling site. It appeared that the motor oil had made its way into the pool of water, and then into the clay tile drain. GonZoil excavated two (2) holes by hand to locate the field tile outside the drill site location. OEPA had straw bales placed around the second, (farthest) excavated area and approved that work be suspended until Sunday, 9/25/2011, due to darkness. On Sunday, four (4) additional holes were excavated to locate tile. Eventually, the end of the clay field tile was located in what appeared to be a French drain at the southern corner of the property between Tremblay and Benes. There was a second tile that emptied into this French drain from the west. The drain tile was flushed from the north to the south, with all the water collected in this French dr

Site is being constructed to requirements under best management practices for well site construction. I was accompanied by Mr. Richard Bayless from the Columbus office. I did a general well inspection of the well location and found no violations. Well is in compliance.

The well head is fenced with chain link fencing with slats and no leaks or problems noted. The tank battery is shared with the KRAL 2-D and has four 140 Barrel lowboy tanks and is enclosed inside chain link fence with slats. Tank are equipped with vents and caps and all are secure, There is a charcoal filter installed, a separator unit and two (2) pick up lines which are locked and secure. There is grass growing and landscaping consists of pine stock on the dike and the dike is sufficient to hold containment for one tank of fluid. Preliminary Restoration passed.
1724438503 2017-02-01 UrbanProduction Wells No Violations I inspected this producing well as part of routine inspection activity. At the time of this inspection I observed the following: The well is part of a three well cluster located in a a mostly wooded parcel. The unpaved access road and areas in and around the well were mostly snow covered, but were in good condition with no signs of erosional deficits or degradation. The production system was comprised of a pump jack and tank battery in separate board-on-board enclosures located in close proximity. The tank battery also serves the two other producing wells on this parcel (Kral 2-D and the Kral 3-D). The enclosure gates were closed and locked. The pump jack was in operation with no evidence of problems or leakage. I walked the interior of the tank battery enclosure and inspected the tanks and associated fittings, connections, and pipe runs. I did not observe any signs of leakage or spills. The tank load line valves were plugged and locked. The tank lids were closed. I walked the areas surrounding the wellhead and battery and found no evidence of impact or degradation.
1768506462 2013-08-30 UrbanProduction Wells No Violations I met with the land owner at 8490 Wyatt Rd. pursuant to an earlier incident in which a small release of oil from a flow-line associated with this well impacted this landowners property near the rear where the property adjoins the parcel on which the well is located. Subsequent to the release, the problem with the flow-line has been resolved and the surface impact from the oil has been remediated. The landowner has indicated satisfaction with the resolution. I also spoke with Wade Balser, Ohio EPA On-Scene Coordinator regarding the previous incident and he indicated that the matter had been resolved to the satisfaction of the OEPA.
