The well and tank battery are located in a wooded area behind the house at the referenced address. The well is comprised of a pump jack system in a board-on-board enclosure. The battery is comprised of 1 100-barrel steel production tank, 1 50-barrel steel brine tank, and mechanical separator. Fittings and connections at the wellhead and battery are secure and free from signs of leakage or problems. Load-outs are secure. Gates are locked. Process safety equipment is present. The access road is good, passable condition with no washouts or other erosional deficits or signs of impact.

The well is located in the rear portion of the single family residence and accessed via a shared lease road which extends to the north past the house and garage. At the time of this inspection, the lease road is in good condition and passable. The well and battery are in separate, board-on-board enclosures which are locked and secure. ID signage is present and accurate. Load-lines at the battery are locked and secure. The secondary containment is intact and there is some ponded storm water in places. There are no signs of leakage or issues. The tank lids are properly secured. Carbon filters, low-pressure relief valves, and spark arrestors are present. The 8.625-inch casing gauge indicates no backside pressure at the time of this inspection. Fittings and connections at the wellhead are secure. A pile of dirt which I had observed at the edge of the pad area previously has been removed. In addition, an area which I had observed to be devoid of grass (possibly from brine scald) at the fringe of the well area has been re-seeded and is now grassy and restored. I did not observe any problems at the time of this inspection.

1203494915 2015-05-07 UrbanProduction Wells Violations Noted I was on site to conduct a status check of this well with DOGRM Inspector - John Fleming and District Manager - Rick Anderson. The wellhead is equipped with a pumpjack and the valves on the wellhead were open. Identification signs are present on the tank battery and wellhead fences, but are not urban rule compliant. The tank battery consists of 1-140 BBL production tank, 1-100 BBL production tank, a mechanical separator, and a 1500 gallon poly brine tank. The production tank sealable hatches are not secured properly. The vent stacks are equipped with pressure relief valves, but no lightning arrestors. There is a charcoal filter. The tank pick-up line valves have flat plugs but are not locked. The pick-up lines have oil staining at the base and are not locked. The well and tank battery were fenced with 8-foot board-on-board fencing, and the man doors were locked. The lease road is stable and drivable. The following needs completed by:

1) Clean-up all spilled oil/brine in tank battery and remove any containers containing fluid (6-16-15) 2) Insert plugs/or add locks into the tank valves and pick-up lines (6-16-15) 3) Install lightning arrestors on vent stacks (6-16-15) 4) Add additional identification sign information as required by 1501:9-9-05(E) (6-16-15)

The access road is concrete for the first 150 and then packed crushed stone for the remainder of the road. The well head is enclosed inside a board on board fence and is secured with locks. The wellhead area is clear, clean and free of debris. The tank battery is inside board on board fencing placed on top of the dike surrounding the battery. There are two 140 BBL storage tanks and one 35 BBL Brine tank. The caps on the tanks are closed and secure. There is a vent system in place for the tanks. A charcoal filtering system is in place. There is one horizontal and one vertical separator in the battery. Load lines are located in the tank battery and are secured with bull head plugs. There is a metering run located in the battery. Landscaping and restoration is complete. Restoration Inspection Passed.
