The spacing within the tank battery and between the unit elemetns is in accordance with 1509:9-9-05 . Unit identification is present and legible at the tank battery. The tank battery secondary containment is adequate. I did not observe any leaks, spills, or other signs of contamination. The gates are locked.

Operator had the hole loaded with fluid and the had dammed off the flow ditch and filled it with drilling fluid.

The wellhead and battery are located near the rear portion of a church property. The wellhead plunger lift and battery are in separate board-on-board enclosures which are locked. Fittings and connections are secure and free from signs of leakage or problems. The load-outs are secure, plugged, and locked. Process safety gear is present. Signage is in place and current. At the time of this inspection there are 15 to 20 inches of snow cover in the area which obscures some surficial elements from observation.

Hole #3 7.875 inches in diameter, total depth 3510 feet. 4.5 inch casing set at 3447.50 casing was new R-3 11.6 lb pipe. Top of the Lockport was 2812. Cement fill up needed 798 feet, 798 X .2278 / 1.61= 112.9 sacks of cement. Operator ran 200 sacks of Unitropic 10-4-2 cement. Operator broke circulation and conditioned the hole with 5 joints of pipe remaining to be ran. Operator completed running pipe, broke circulation and ran cement. Operator displaced cement and locked down plug with 1500psi. No circulation issues during displacement.

At the time of this inspection the wellhead and battery board-on-board enclosures were closed and locked. The load-line valves were secure and locked. Tank lids were closed and secure. Secondary containment as intact and free from signs of release, leaks, or problems. The well was in active production. Production reports were current. Bonding and insurance were active and current. I walked and inspected the areas surrounding the wellhead and battery and found no evidence of impacts from leaks, releases, or degradation to the natural greenscape.

1943150093 2017-03-15 UrbanProduction Wells No Violations I inspected this well location as part of routine inspection activity. At the time of this inspection, I observed the following: a wellhead with plunger lift system and associated tank battery located adjacent to the parking area of the active church facility in separate board-on-board enclosures which were locked. Access to the battery and wellhead was afforded by a short, access road which was snow-covered, but accessible. The battery was comprised of one 100-barrel steel low-boy production tank, one 50-barrel steel brine tank, and horizontal separator. The fittings and connections at the wellhead and battery were secure and free from signs of leakage. The load-lines were plugged, locked, and secure. Process safety equipment was present, and the ID signage was present and accurate.
