0818631842 2017-06-15 UrbanProduction Wells No Violations I inspected this well location along with DOGRM Asst. Supervisor Stephen Ochs as part of routine inspection activity. The gravel-paved access road to the wellhead and battery was dry, and in good condition. The wellhead pump-jack system was in operation. The fittings and connections were secure. Secondary containment at the battery was intact and dry. I walked the areas around the well and battery and found the natural greenscape to be free from signs of impact. Production reports were current and there were no other problems or issues noted.
1111933418 2014-11-21 UrbanPreliminary Restoration No Violations This well has recently been completed and placed into production. The well is comprised of a pump jack system in a chain-link enclosure. The tank battery is comprised of 1 100-barrel steel production tank and 1 100-barrel steel brine tank, with separator unit in a chain-link enclosure. The battery is located within the same footprint in a clearing of the wooded area east of the career center. The well and battery are accessed via a gravel-paved road which travels east from the career center parking lot. There is a locked gate at the head of the lease road which has ID signage. The lease road is in good condition with no erosional deficits. The former drilling pits have been closed but the area lacks final grading for preliminary restoration. Ive discussed with Moore Resources and agreed that preliminary restoration will be completed in the spring due to early snowfall.

Fittings and connections at the well were secure and free from signs of leakage or problems. The chain-link enclosure was locked and secure. The associated tank battery is located in a separate chain-link enclosure which was locked and secure. Tank lids were closed and secure. The secondary containment was intact and dry. Load-lines were locked and secure. Signage was present. I walked the areas surrounding the well and battery and found no signs of degradation or impact to the natural greenscape.
1489960766 2014-07-15 Drill / Deepen / Reopen No Violations I performed a status check as part of on-going drilling activity and test of blowout preventer (BOP). The BOP was installed and pressured up at 6 PM. The maximum test pressure was observed to be 200 PSI. Pressure was held for 30 minutes and the test was concluded with no observed problems.
