0515847396 2015-06-22 UrbanProduction Wells No Violations I met with Mayor Bob Stefanik at the well location in response to a complaint registered from a neighbor regarding observed oil sheen downslope from the referenced well. Reportedly, there has been re-current and chronic issues with drainage on the referenced property. At the time of this visit, I observe some areas where surface water is ponding in the grass. Some of these areas appear to be draining poorly. Reportedly, at the time that the well was completed, the operator (Cutter Oil) installed a drainage connection between the gravel drive and a drainage catch basin 80 feet east of the edge of the driveway, hereinafter referred to as CB#1. That catch basin is believed to connect to a 2nd catch basin located 110 feet further east near the eastern property boundary, hereinafter referred to as CB#2. CB#2 drains to the west side Foxwood Drive tree lawn, via an 8-inch PVC pipe which travels east and terminates as an open pipe under a shrub. The tree lawn area around the terminus is saturated with effluent storm water. Drainage from this area is to the south towards a 3rd catch basing (CB#3). Surface water is also seen to be traveling over the sidewalk and into Foxwood Drive. The landowners (Dellavalle) report that there have been historical problems with drainage at the property and attribute some of this to the presence of the well.

A neighbor reported seeing oil sheen in the water ponded along Foxwood Dr. Sheen was also noted in the area of CB#1 and near the wellhead. At the time of this inspection, I observed sheen on surface water at the following locations: on ponded water on the east side of the wellhead, on ponded surface water near CB#1, and on ponded surface water along Foxwood Dr. Upon close inspection, I noted the sheen to break up and fracture on physical contact. There is no petroleum odor. These observations suggest that the sheen is related to iron bacterial activity and not associated with crude oil or other petroleum source.An inspection of the well

0667100887 2019-05-06 Production Wells No Violations I conducted an inspection of the well and found no violation at well or common tank battery with the Dellavalle 2D.
   The following maintenance issue needs to be corrected.

Gaps between the top of the dike and bottom of the board on broad fencing around the dike. Fluid has collected within the Dike area needs to be removed and disposed of properly. Charcoal filter needs replaced due to bottom rusted out. I contacted Rick Little and he stated the fluid will be pumped out and disposed of and the charcoal filter, fencing issues it will be put on the repair schedule.

This is a vertical well and shares a tank battery with the Dellavalle 2 D well. Pressure at the annulus was 150 PSI. The tank battery is shared with Dellavalle 2D and each well has a separator, 2 (Two) 100 Barrel tanks and 1 50 barrel brine tank. There is one sales line for both wells. No violations were found. Well is in compliance.

1437656373 2015-07-22 UrbanProduction Wells No Violations I met with Northeast Region Supervior Tom Hill at this location as part of on-going investigation of complaint relating reported sheen along Foxwood Dr. At the time of this inspection, the areas of standing water in the grass along the west side of Foxwood Dr. are wet but with less stagnant water. We observed some reddish iron staining on the sidewalk in areas where the sheen and baterial slime effluent have been previously observed. We performed Quantab analysis at the catch basin on the Dellavale property and at the secondary containment drains at the Dellavalle 1 and 2, and the tank battery. Chloride readings ranged from 200 to 450 mg/L at the wellhead area and the storm inlet, respectively. We did not observe any signs of stressed vegeatation in the areas where sheen has been reported.

At the time of my inspection, I found the following: wellhead with pump jack system and associated battery in separate board-on-board enclosures. The enclosure gates were locked and secure. The fittings and connections at the wellhead and battery were secure and free from problems. The tank lids were closed and secure. The interior of the battery, including secondary containment was free from free oil, staining, or other indications of spills, leaks, or problems. The battery is shared with the Dellavalle #2D, located nearby. The electrical panels and load line valves were locked and secure. I walked and inspected the areas surrounding the well and battery and found no indications of impacts or problems relating to the on-going production activity. The shared-use access road was snow-covered by accessible.
1643302669 2015-05-07 UrbanProduction Wells Violations Noted I was on site to conduct a status check of this well with DOGRM Inspector - John Fleming and District Manager - Rick Anderson. The wellhead is equipped with a pumpjack and the valves on the wellhead were open. Identification signs are present on the tank battery and wellhead fences, but are not urban rule compliant. The belt guard for the pumpjack was on the ground next to it. There was an open 5 gallon bucket full of oil next to the wellhead. The tank battery consisted of 4-140 BBL production tanks, 2 mechanical separators, and 2-1500 gallon poly brine tanks. The production tank sealable hatches are not secured properly. The vent stacks are equipped with pressure relief valves, but no lightning arrestors. There is a charcoal filter. The tank pick-up line valves have flat plugs but are not locked. The crossover line between the production tanks is leaking. The brine tank is seeping at the valve connection and the bleeder line has oil staining under it. The pick-up lines have oil staining at the base and one is leaking, and none are locked. The well and tank battery were fenced with 8-foot board-on-board fencing, and the man doors were locked. The lease road is stable and drivable. The following needs completed by:

1) Clean-up all spilled oil/brine in tank battery and remove any containers containing fluid (6-16-15) 2) Insert plugs/or add locks into the tank valves and pick-up lines (6-16-15) 3) Install lightning arrestors on vent stacks (6-16-15) 4) Add additional identification sign information as required by 1501:9-9-

At the time of this inspection, I noted the following: a wellhead with pump-jack system and associated tank battery in separate board-on-board enclosures located on a residential property. The well is co-located with the #2-2181 well. Fittings and connections at the wellhead were secure. The pump-jack supports were solid. The stuffing box fitting was secure with no leaking or problems. The bridle attachments were secure. Fittings and connections at the battery were secure. The load-line valves were plugged and locked. The tank lids were closed and sealed. The secondary containment had several inches of standing water from recent rainfall. The water was clear and free from any signs of spills, leaks, or other problems. Access to the two wells and associated battery is from a shared-use, gravel-paved access road that enters from Wallings Road and also provides access to a barn and residential structure. I walked the areas surrounding the well and battery systems and observed the natural soils and vegetation to be free from signs of leaks, spills, or other adverse impact from the on-going production activity.
