0146977530 2012-11-27 UrbanPreliminary Restoration No Violations On my visit to this well today I was accompanied by Steve Tomkins, Lake County Inspector DOGRM. The purpose of the inspection was for Urban Preliminary Restoration. The access road consists of crushed limestone and brick packed to a depth of six inches. Access road is secured by a cable across the entrance and is secured and locked. The wellhead is enclosed by a board on board fence and there are no leaks or problems and is secured with locks. The tank battery is a shared unit with the Oyler 1 D well P# 2-2003 and the Oyler 2 D well P# 2-2125. There are a total of six tanks in the unit and each well has their own tank units. There are two 140 BBL tanks and one 70 BBL Brine tank for this unit. There is one vertical and one horizontal separator for the well with load lines and a metering run. Load lines are secured with locks and bullhead plugs. The tank hatches are closed and secure with vent pipes and a charcoal filtering unit. Tank battery is neat and clean with no signs of leakage or debris. Preliminary Inspection passed.

The unit is producing and production reports are current. ID placards are present and have current and accurate contact information. The tank battery secondary containment dike is is good condittion. The natural surface drainage from the area of the wellhead and tank battery is to the southeast and I did not observe any signs of gully formation, wash-outs, or other impacts associated with the unit. Enclosure gates are locked. I did not observe any signs of leakage, spills, or other problem conditions.

The wellhead and battery are located in a wooded area on the south side of Tollis Parkway. The wellhead pump jack and battery are in separate board-on-board enclosures which are locked. Signage is present. Secondary containment at the wellhead and the battery are present and well maintained. Fittings and connections at the wellhead and battery are secure and free from signs of leakage or problems. The battery is shared with the #2-2125 and the #2-2003 units. The load-outs are plugged. Gates are locked.

1156349837 2015-05-07 UrbanProduction Wells Violations Noted I inspected this well location as part of routine inspection activity. The well is located in a clearing within a primarily wooded area. The entry to the well is gated with a cable-style gate which is locked. . The access road is gravel-paved and accesses each of three wellheads (Oyler 1, 2D, and 3D). At the time of this inspection, there are no wash-outs or erosional deficits. The wellhead is comprised of a pump jack unit in a board-on-board enclosure which is locked. The shared tank battery is comprised of a total of 6 140-barrel steel production tanks, 3 50-barrel poly brine tanks, and 3 mechanical separators. The entry door is secure and locked. At the time of this inspection, the following maintenance issues were noted: Electrical boxes are not locked. Production tank lids are not latched down as required. The dike drain riser is not secure and leaking which requires remedy. There is a significant gap between the bottom of the fence enclosure and the ground surface at the well enclosure and battery rendering the enclosures unsecure. Signage is present but lacking complete information. There is discarded rod clamps and other refuse in the wellhead enclosure which should be properly managed. An empty 55-gallon drum and chemical pump should be removed if not used. The 8.625-inch casing is not exposed for monitoring as required. In addition, the following safety issues were noted. Load lines are not locked. Brine is actively leaking from the 2-inch flange at the brine tank. These safety issues must be remedied by 06/19/2015. Please contact Inspector Stephen Tompkins with questions regarding these issues.

1445208862 2015-10-16 Plug / Plug Back No Violations I was on location pursuant to notification of planned plugging and abandonment activity. Prior to my arrival, crews from the owner/operator had dismantled the pumping unit and removed the sucker rods and production tubing. Residual pressure had been relieved and the well was open to atmosphere and stable with no appreciable gas being produced. Plugging and abandonment activity were executed in compliance with OAC 1501:9-11. Plugging activity is detailed in the associated plugging report. Upon establishment of the top surface plug, the well was noted to be stable with no gas production. A steel plate with well ID had been prepared and will be secured to the wellhead prior to burial at 30 inches below grade as required. Final restoration activities will be on-going until completion as required. The well shares a tank battery, sales line and other production appurtenances with the Oyler 1 & 2 units. These shared elements will remain as part of the production systems associated with those units.
