The well and associated batery are located on an otheriwse undeveloped parcel at the corner of Karen Lynn and Mill Rd.. At the time of this inspection, I observed the following: a wellhead with pump jack system and associated tank battery in separate chain-link enclosures. Enclosure gates were locked. Load-line valves were plugged, locked and secure. Fittings and connections at the wellhead and battery were secure and free from signs of leakage. The secondary containment was intact.

The enclosures are locked. Signage and ID is present and in good condition. There is secondary containment at the wellhead and battery. The well is producing with no signs of leakage or problems. The battery is comprised of 1 steel 100-barrel production tank and 1 steel 100-barrel brine tank, and separator unit. Process safety equipment is present. The dike is free of oil. The load-outs are locked and are fitted with secondary containment. The access drive has a cement apron, and surrounding areas are well maintained and runoff appears to be well managed. I noted that the gate valve for the dike drain in broken, leaving it free to drain to the adjacent creek. I contacted well tender Bob Allen about it, and he indicated repairs would be made today.

Mill and Karen Lynn, on an otherwise undeveloped parcel. Access to the well is afforded by a short, concrete-paved access road which enters from Karen Lynne. At the time of this inspection, I observed the following: enclosure gates are locked and secure. Tank hatch lids are closed and sealed. Load lines are lock-protected and secure. The electrical box is locked. Fittings and connections at the wellhead and battery are secure and free from signs of leakage. Process safety equipment is present. The secondary containment is intact and free of staining. The ID signage is not accurate and not compliant with OAC 1501:9-9-05(E)(1)(a-e). The 8.62-inch casing annulus is not plumbed to surface and gauged as required by a condition of the permit. I notified the well owner about these deficiencies. The owner has reported that new signs are being prepared.

The unit is producing and no problems observed.
1605897601 2016-05-09 UrbanProduction Wells No Violations I inspected this well location pursuant to a complaint (5318) of possible impact to a domestic water well located within 1/4 mile of this location. At the time of this inspection, I observed the following. A wellhead with pump jack system and tank battery is separate chain-link enclosures which were locked and secure. The fittings and connections at the wellhead and battery were secure. Load-lines were plugged, secure, and locked. Tank lids were closed and secure. (signage, surface casing, and lock on electrical box). Secondary containment was intact at the battery. The drain was closed. I walked the area surrounding the wellhead and battery and did not observe any signs of leakage, impact, or problems associated with the well. I noted that the surface casing nipple was not exposed and open or gauged as required by permit condition. I contacted Bruce Levengood with Sound Energy and advised him about the surface casing permit condition requirement. He indicated the condition would be corrected within 48 hours. I met with well tender Bob Allen at the location the following day, and the casing was property plumbed to surface.
