1012369232 2013-10-10 UrbanProduction Wells No Violations The producing wellhead includes a pump jack in a 8 board-on-board enclosure. The aboveground storage tank (AST) battery, which is shared with the Glatzer 2D unit, has been expanded to include a total of 2 100-barrel steel production tanks, 2 25-barrel poly brine tanks, 2 separators and a metering run. The battery is enclosed in an 8 board-on-board fence. The gates are locked. Spacing, unit ID signage, and secondary containment are good. Areas disturbed during drilling, including pits, have been filled, re-graded and seeded. The short access road to the wellhead has been graveled. Process safety equipment (spark arrestor, low-pressure relief valve, and carbon filter element) is present. No problems observed.
1238764542 2009-03-26 Drill / Deepen / Reopen No Violations Wildcat is drilling ahead. The pits are lined and in good condition and a tarp is covering the end of the blow line.
1369844340 2013-05-28 UrbanProduction Wells No Violations The producing unit consists of the following elements: a wellhead with pumpjack in a board-on-board enclosure, and a tank battery system also in a board-on-board enclosure. The enclosures are secured with locks and in in good condition. The tank battery is comprised of two 100-barrel steel tanks, a 25-barrel poly tank, and mechanical separator. HB 278 safety equipment is in place. I did observe the pressure lid on one of the production tanks was open and gas was escaping. I spoke with Ted Hill of Ohio Valley Energy and he indicated that he would fix it immediately. I did not observe any other problems or violations.
1679730291 2009-04-06 Drill / Deepen / Reopen No Violations working on location
2031305534 2014-09-22 UrbanProduction Wells No Violations I inspected this producing unit as part of routine inspection activity. There is currently a wellhead with pump jack and tank battery which shared with the Glatzer 2D. I observed oil leaking from a pressure relief fitting at the pump jack and some oily impact from leaking fittings in the tank battery. I contacted Rick Liddle with Ohio Valley Energy and advised him of the need for mechanical attention and clean-up of oil at the battery and wellhead. He indicated crews will be tending to these issues within the week.
