1708425946 2011-12-28 UrbanPreliminary Restoration No Violations On 12/28/2011, DNR Inspector Richard Skidmore completed an urban preliminary restoration well inspection for DNR Inspector Ray Wallace. The lease road, tank battery, and well head locations are in compliance with 1501:9-9-05. Inspection passed.
1760117586 2014-08-19 UrbanProduction Wells No Violations I inspected this producing unit as part of routine inspection activity. The well was completed in 12/22/2008. The unit is producing and production reports are current. The well is located in a upscale suburban area with large lots, and densely wooded areas around the well and battery. The producing unit is accessed via an extension from the existing residential driveway. The access road is primarily grassy lawn with no gravel or other pavement. Elements of the well consist of a production wellhead with standard valve assembly and mechanical plunger-lift system and aboveground storage tank (AST) battery in separate board-on-board enclosures. The valve fittings, connections, and other production appurtenances appear to be well maintained and are free from signs of leakage or other problems. The battery is comprised of 1 steel 140-barrel production tank, 1 50-barrel poly brine tank, and separator unit. Spacing from well to battery, and spacing of elements within the battery meet regulations. The load-outs are bull-plugged. There is secondary containment at the battery and the interior areas are dry. I did not observe any signs or evidence of leakage, spills, or problems. Unit identification placards are present at both the battery and wellhead and are in good condition. I observed no issues or problems requiring attention at the time of my inspection.
1788945938 2016-02-02 UrbanProduction Wells No Violations I inspected this well location as part of routine inspection activity. At the time of this inspection, I observed the following: Ingress and egress from the well area is afforded by the shared driveway for the single-family residence. A wellhead with mechanical plunger lift system is located in a board-on-board enclosure in the backyard. The well enclosure is locked and secure. Fittings and connections at the wellhead were secure and free from leakage. The 8.625-inch casing annulus was exposed and fitted with pressure-relief fitting. The wellhead enclosure is surrounded with pine-tree style evergreen screening which was noted to be in good condition. Areas around the wellhead consist of turf grass which was free from signs of stress or impact. The tank battery is located 50 feet west of the wellhead within the same grassy yard area. The battery is comprised of one 100-barrel, low-boy steel production tank, one 50-barrel poly brine tank. At the time of this inspection, the access gate to the battery is locked and secure. Signage was present. Fittings and connections were secure and free from signs of leakage or problems. Process safety gear and carbon filter system was present. Secondary containment was intact and dry. Tank lids were closed. The load-outs are located remotely from the battery, at the edge of the gated portion of the yard and were plugged and locked. Evergreen screening was present surrounding the battery and in good condition. I walked the area surrounding the battery and observed normal grassy vegetation with no signs of erosional deficits or impacts associated with the production operation. I did observe the following maintenance issue: the production tank lid is missing the closure wing-nut. This needs replaced to effect a proper seal. I notified the well owner about the need to replae the missing wing nut.
